What the Policy Questionnaire Template Is

A Policy Questionnaire Template is a standardized form used to collect consistent, verifiable information about an individual, entity, or set of risks when applying for an insurance policy, onboarding a vendor, or documenting internal compliance. It centralizes personal and organizational data, coverage preferences, prior-claim history, and supporting details so underwriters and risk managers can evaluate exposure and price coverage accurately. Templates reduce manual entry, speed review cycles, and provide an auditable record of the facts submitted with an application or renewal request.

Why a Structured Questionnaire Matters

A consistent Policy Questionnaire improves accuracy, streamlines underwriting, and creates a defensible record of the facts provided during application or renewal for compliance and audit purposes.

Why a Structured Questionnaire Matters

Who Typically Completes This Template

Clear completion improves turnaround and reduces follow-up requests for missing or inconsistent information.

  • Risk managers and underwriters who evaluate exposures and determine coverage eligibility.
  • Insurance agents or brokers preparing applications and supporting documentation for carriers.
  • Applicants or policyholders providing personal, business, or claims-history details for underwriting.

Who Can Sign or Authorize the Questionnaire

Corporate Officer

An authorized corporate officer (CEO, CFO, COO) may sign when the applicant is a business; the signer must have authority to bind the entity and confirm accuracy of disclosures in writing.

Licensed Agent

A licensed insurance agent or broker may complete and submit the questionnaire on behalf of a client if the client expressly authorizes the agent and the submission documents the agent’s capacity and contact information.

Step-by-Step: Filling Out the Questionnaire

Follow these steps in order to prepare a complete, reviewable questionnaire for underwriting or compliance review.

  • 01
Gather Documents: Collect IDs, loss runs, business filings, and supporting exhibits.
  • 02
Complete Fields: Enter required data, use MM/DD/YYYY dates, and attach files where prompted.
  • 03
Review for Accuracy: Verify names, addresses, and numerical entries for consistency.
  • 04
Sign and Submit: Have an authorized signer apply signature and provide contact details for follow-up.

Typical Submission and Review Flow

A concise view of how questionnaires move from completion to policy decision and recordkeeping.

  • Upload: Applicant uploads completed form and attachments to carrier portal.
  • Validation: System or staff check required fields and flag missing items.
  • Underwriting: Underwriter reviews the facts, requests clarification if needed.
  • Decision: Carrier issues quote, amendment, or declination with documented reasoning.

Essential Sections Every Professional Questionnaire Should Include

Applicant Details

Legal name, DBA, EIN or SSN as applicable, business type, and full contact information so identity and authority can be verified against public records and tax filings.

Operations Summary

High-level description of activities, locations, and employee counts to frame exposure and determine jurisdictional licensing or regulatory requirements.

Risk Exposures

Specific risk factors, safety controls, and mitigation measures with dates and descriptions to support accurate rating and endorsements.

Claims and Loss History

Complete prior-claim listing with dates, amounts paid, and status; attach carrier loss runs for full claims validation.

Coverage Selections

Requested limits, deductibles, and optional endorsements to enable immediate premium calculation and identify coverage gaps.

Supporting Attachments

Policy declarations, inspection reports, contracts, or financial statements that provide evidence supporting underwriting conclusions and rating decisions.

Required Fields Snapshot

Policyholder Name: Full legal name
Tax ID: EIN or SSN
Contact Phone: Direct number
Email Address: Valid company or personal email
Effective Date: MM/DD/YYYY
Signature: Typed or signed

Consequences of Inaccurate or Incomplete Questionnaires

Claim Denial: Possible refusal to pay covered losses
Policy Rescission: Carrier may void policy ab initio
Premium Adjustment: Retroactive increase in premium
Regulatory Fines: State penalties for false statements
Backup Withholding: Tax treatment if incorrect TIN provided
Contract Risk: Disputed coverage due to misstatements

Common Preparation Pitfalls to Avoid

  • Incomplete prior-loss details that force underwriters to request loss runs and extend review timelines.
  • Using inconsistent names or addresses between supporting documents and the questionnaire, which triggers identity verification delays.
  • Attaching scanned low-resolution documents that cannot be read by reviewers or OCR tools, causing re-submission requests.
  • Skipping required authorizations or consumer disclosures where ESIGN consumer disclosure rules apply for paper-to-electronic conversion.

Key Processing Milestones

Sequential milestones from submission through decision; each stage has typical target times for high-volume operations.
