SCOPE OF WORK

pFMEA WORKSHEET

Instructions: For each process step, identify potential failure modes, potential effects, assign Severity (S), Occurrence (O), and Detection (D) ratings (1-10), and compute the Risk Priority Number (RPN = S × O × D). Record current controls, recommended actions, responsible party, target completion date, and verification status.

# Process Step Potential Failure Mode Potential Effect(s) S Potential Cause(s) O Current Controls D RPN Recommended Action(s) Responsible Target Completion Verification / Status

RPN Calculation Note: RPN (Risk Priority Number) is computed as Severity × Occurrence × Detection. Use this value to prioritize recommended actions. High RPNs require documented corrective or preventive actions and assignment of responsibility.

What the Process Failure Mode and Effects Analysis (pFMEA) Template Is

The Process Failure Mode and Effects Analysis (pFMEA) Template is a structured worksheet used to identify, prioritize, and mitigate potential failure modes in a manufacturing or service process. It captures process steps, potential failure modes, causes, current controls, severity, occurrence, and detection ratings, and documents recommended actions with ownership and due dates. The template supports cross-functional risk assessment, continuous improvement, and traceable mitigation tracking to reduce defects, safety incidents, rework, and downtime across operational workflows.

Why a pFMEA Template Matters for Process Risk Control

A consistent pFMEA template helps teams detect high-risk process steps early, prioritize countermeasures, and assign responsibility. It creates a record of decisions and reduces variability in quality reviews, regulatory audits, and supplier evaluations.

Why a pFMEA Template Matters for Process Risk Control

Who Typically Completes a pFMEA and How Teams Collaborate

Cross-functional teams—operations, quality, engineering, and process owners—use pFMEA to document risks and corrective actions before changes or launches.

  • Quality engineers and auditors who lead risk scoring and verify corrective action effectiveness during audits and continuous improvement cycles.
  • Process engineers and operations leads who map steps, list potential causes, and propose mitigation measures tied to equipment or SOP changes.
  • Suppliers and procurement when onboarding new components or services, to ensure upstream controls and prevent downstream process failures.

Use the template as a living document during process design, pilot runs, and periodic risk reviews to keep controls current and effective.

Core Sections to Include in a Professional pFMEA Template

A robust pFMEA template organizes information so teams can assess severity, occurrence, and detection, calculate risk priority numbers, and track corrective actions to closure.

Process Steps

Numbered sequence of discrete process actions or operations with clear start and end points to focus failure analysis on specific activities.

Failure Modes

Concise description of how a process step could fail, expressed in terms that are testable or observable to guide mitigation design.

Causes

Document underlying causes for each failure mode, including material, machine, method, measurement, manpower, and environmental factors.

Current Controls

List existing prevention and detection controls such as inspections, interlocks, training, or SPC so teams can assess residual risk.

Risk Ratings

Severity, occurrence, and detection scores with RPN calculation rules and clear scoring guidance to standardize prioritization.

Recommended Actions

Action description, owner, target date, verification method, and closure evidence to ensure accountability and measurable risk reduction.

Step-by-Step: How to Fill Out the pFMEA Template

Follow these sequential steps during a facilitated review session to produce a consistent and auditable pFMEA record.

  • 01
Prepare: Assemble process map, data, and cross-functional attendees before the session.
  • 02
Identify Steps: Break the process into discrete steps and assign IDs for clarity.
  • 03
Assess Risk: For each failure mode, score severity, occurrence, and detection using the approved matrix.
  • 04
Plan Actions: Assign corrective actions, owners, target dates, and verification criteria for RPN reduction.

How to Configure an Online pFMEA Workflow

Configure the digital workflow so reviewers can collaborate, sign off, and receive reminders without losing version history.

Document Template Upload a locked PDF or editable DOCX as the base template so fields remain consistent across projects.
Field Mapping Create required fields for step ID, failure mode, scores, owner, and due date with validation rules.
Reviewer Routing Set role-based signer order and include stakeholder groups for sequential approvals.
Notifications Enable email or in-app reminders for action owners and overdue items with escalation rules.
Audit Trail Enable full audit logs that capture timestamps, actor identity, and field-level changes for compliance.

Typical eSubmission and Approval Flow for a pFMEA

A clear digital routing sequence minimizes delays and preserves a complete audit trail for each review and approval step.

  • Upload Template: Owner uploads the pFMEA file and selects the project or part number.
  • Populate Fields: Team members enter process steps, failure modes, causes, controls, and scores.
  • Review & Sign: Assigned reviewers validate entries and sign electronically in order.
  • Close Actions: Owners update actions to closed with evidence and final verification sign-off.

Technical Requirements for Digital pFMEA Collaboration

Confirm the platform can retain historic versions and export completed pFMEAs with audit metadata for audits or regulatory review.

  • File Formats: PDF, DOCX, and XLSX supported
  • Integrations: Common integrations include ERP and PLM systems
  • Authentication: Support for SSO and multi-factor

Essential Data Fields and Security Considerations

Project ID: Unique identifier
Part Number: Manufacturer part
Owner: Responsible person
Review Date: MM/DD/YYYY
RPN: Severity×Occurrence×Detection
Evidence: Attachment or log

Common Mistakes When Preparing a pFMEA

  • Using inconsistent scoring matrices across teams, which leads to incomparable RPNs and mis-prioritized actions.
  • Documenting overly broad failure modes rather than specific, observable outcomes, making root-cause analysis and controls ineffective.
  • Neglecting to assign clear owners and due dates for recommended actions, causing corrective work to stall or never close.
  • Storing pFMEA versions locally without an audit trail, which complicates audits and obscures when controls were implemented or changed.

Risks of an Incomplete or Incorrect pFMEA

Product Recall: Increased recall risk
Safety Incidents: Potential regulatory fines
Warranty Costs: Higher replacement costs
Quality Nonconformance: Failed audits
Supply Disruption: Production delays
Legal Exposure: Contractual liability

Typical Timelines and Review Cadence for pFMEA Records

Set and track deadlines for initial pFMEA completion, action verification, and periodic revalidation based on risk and product lifecycle stage.

Initial Completion Deadline:

Prior to pilot run or process validation

Action Target Dates:

Owner-assigned with escalation for overdue items

Verification Review:

Confirm effectiveness after implementation

Periodic Revalidation:

Annual or after major process change

Audit Retention Check:

Review storage and access for audits
