What the Producer Change Form Is

A Producer Change Form documents the transfer, replacement, or reassignment of an insurance producer, agent, or broker tied to a specific policy or set of policies. It records the outgoing and incoming producer details, effective date, carrier and agency references, and authorization for commission routing or servicing changes. Carriers, agencies, and brokerages use this form to update internal records, vendor lists, and commission systems while creating an auditable record of who is authorized to represent the insured.

Why maintaining a clear Producer Change Form matters

A properly completed Producer Change Form reduces commission disputes, ensures accurate carrier records, and creates an audit trail for regulatory and tax purposes. It clarifies authority, effective dates, and contact details so carriers and agencies can process changes without interruption.

Why maintaining a clear Producer Change Form matters

Who typically completes or receives this form

The Producer Change Form is used by multiple internal and external stakeholders involved in policy servicing and commission administration.

  • Insurance carrier operations teams who update policy records and apply commission changes.
  • Agency or brokerage administrators who initiate changes and gather producer authorizations.
  • Producers or agents who must sign to accept or relinquish appointment and commission routing.

Each party has distinct responsibilities for accuracy, signatures, and record retention; coordination reduces processing delays.
