The Producer Reregistration Form is a standardized document used by licensed insurance producers, brokers, and registered agents to update licensing, appointment, and contact information with carriers and state insurance departments. It confirms continuing eligibility, records changes in business address or control, and documents required disclosures for compliance. Many carriers use the form during annual renewals, when a producer changes employer, or after material changes such as ownership transfers. The form is typically accepted electronically under federal ESIGN rules and applicable state e-signature statutes when executed with required authentication and retention.

Why Completing a Producer Reregistration Form Matters

Accurate reregistration keeps licensure and carrier appointments active, maintains regulatory compliance, reduces payment or commission disruption, and creates an auditable record for audits and consumer protection.

Who Typically Completes the Producer Reregistration Form

The form is completed by licensed producers, agency administrators, and carrier appointment teams to keep records current and compliant.

  • Licensed Producer — Updates licensing status, addresses, and appointment changes to maintain carrier contracts and commission flow.
