What the Professional Expertise Request Form Is

The Professional Expertise Request Form is a structured written request used to engage, document, and verify the qualifications and scope of work for a professional consultant, expert witness, or specialist retained for a project or matter. It captures requester and expert contact details, scope and timeline, deliverables, fees, confidentiality provisions, conflict-of-interest disclosures, and signature blocks so parties have a clear record of the engagement. This form supports auditability, consistent intake, and downstream invoicing and compliance checks across professional, legal, healthcare, and financial contexts.

Why a Standardized Request Form Matters

A consistent Professional Expertise Request Form reduces misunderstandings, clarifies responsibilities, and creates a reproducible record for review or audit. It helps align expectations on deliverables, fees, and confidentiality while supporting legal defensibility under ESIGN (15 U.S.C. ch. 96) and state electronic transaction rules such as UETA.

  • Procurement and project managers for consultants, auditors, and technical specialists.
  • Healthcare administrators when onboarding contracted clinical consultants under privacy controls.

The requester, hiring manager, and the professional all keep copies; signature authority varies by organization policy.
