Common preparation errors to avoid

  • Incomplete contributor identification—omitting legal entity names, tax IDs, or contact details leads to payment delays, tax reporting errors, and difficulty verifying authority to contribute.
  • Vague contribution descriptions—using non-specific terms like 'services' without hours or deliverables prevents proper valuation and may cause disputes or audit findings.
  • Missing supporting documents—failure to attach invoices, timesheets, or photos undermines valuation and complicates reconciliation with accounting records during audits.
  • Unsigned or improperly signed forms—use of initials only, missing titles, or unsigned electronic records without recorded consent can invalidate the form in compliance reviews.

Key risks and potential penalties

Tax Consequences: Backup withholding at 24% if TIN missing.
Reporting Penalties: Incorrect reporting may trigger IRC §6721 penalties.
Audit Exposure: Insufficient records increase audit risk.
Contractual Breach: Unclear terms can cause breach claims.
Reputational Risk: Donor disputes can harm public trust.
