What the Project Evaluation Go-No Go Form Is

The Project Evaluation Go-No Go Form is a structured decision document used to record the assessment, risks, costs, and approvals required to move a project forward or stop it. It captures project scope, key success criteria, estimated budget, risk register, recommended decision (go or no go), and required signatories so the organization has an auditable record of the decision and its rationale.

Why this form matters for project governance

A consistent Go-No Go Form centralizes decision criteria, reduces ambiguity across stakeholders, and creates an auditable trail for governance, procurement, and compliance reviews.

Why this form matters for project governance

Who typically completes and reviews the form

The completed form serves as the official record of the decision and the basis for next steps, whether execution, rework, or closure.

  • Project managers and business analysts — compile project facts, costs, schedule, and initial risk scoring for reviewer assessment.
  • Risk and finance reviewers — validate assumptions, check budget alignment, and confirm financial feasibility before approval.
  • Executive sponsors and governance boards — weigh strategic alignment and provide the final go/no-go decision and required signatures.

Authority and signatory roles

Project Sponsor

The Project Sponsor is the senior executive accountable for the initiative. This individual reviews the cost-benefit analysis, strategic fit, and residual risks then signs to authorize funding or to decline the investment, ensuring executive-level accountability for the outcome.

Authorized Signatory

An authorized signatory (such as a VP of Finance or Program Director) confirms that budget, contract terms, and compliance checks are satisfied. Their signature attests to funding availability and operational readiness as required by internal control policies.

Core sections to include in a professional Go-No Go Form

A complete form captures project identifiers, decision criteria, quantified risks, financials, stakeholder approvals, and an audit trail to support the decision and future reviews.

Executive Summary

One-page synopsis of objectives, outcomes sought, and the single recommended decision so reviewers can quickly determine strategic fit and urgency.

Decision Criteria

Clear, measurable gates (cost, schedule, scope, risk thresholds, regulatory requirements) that the project must meet to receive a 'Go' recommendation.

Financial Summary

Estimated capital and operating costs, funding source, ROI assumptions, and any contingency reserves or alternate funding scenarios.

Risk Register

High-level risks with likelihood, impact, mitigation plans, and residual exposure to inform the final determination.

Approval Routing

Named approvers, signing order, and any required supporting reviews (legal, compliance, procurement) to complete before final signature.

Audit Trail

Record of submissions, reviewer comments, timestamps, and final signatures to support later audits or project retrospectives.

Common mistakes to avoid

  • Incomplete financial assumptions cause repeated reviewer questions and delay approvals; include supporting spreadsheets or source numbers.
  • Unclear decision criteria lead to subjective outcomes; define pass/fail thresholds and scoring rubrics in advance.
  • Missing stakeholder signoffs create governance gaps; list required reviewers on the form and enforce routing order.
  • Using inconsistent project identifiers prevents matching records across systems and complicates audits and reporting.
