What the Project Management FCR Template Is

The Project Management FCR Template is a standardized Field Change Request form used to document proposed alterations to project scope, schedule, budget, or technical specifications. It captures requester details, a clear description of the change, impact assessments (time, cost, quality), proposed implementation steps, and required approvals so stakeholders can evaluate, route, and record decisions consistently across a project lifecycle.

Why a Formal FCR Template Matters

A consistent FCR template reduces ambiguity, helps quantify schedule and cost impacts, and creates an audit trail for approvals and rollbacks. It improves governance of scope changes and supports accurate recordkeeping for compliance and claims.

Why a Formal FCR Template Matters

Who Typically Prepares and Reviews an FCR

Project teams and stakeholders complete FCRs to propose, review, and authorize changes so decisions are documented and actionable.

  • Project managers and coordinators who identify or log scope and schedule changes for review.
  • Contractors and subcontractors proposing field or scope adjustments with cost and time impacts.
  • Clients, sponsors, or change control boards who review authorization, budget, and contractual implications.

Proper role alignment ensures timely review, clear approvals, and minimal disruption to downstream work.

Core Sections to Include in a Professional FCR

A well-structured FCR groups information so reviewers can understand the request quickly, assess impacts, and record approvals for future reference.

Change ID

Unique identifier and versioning to track revisions and link approvals to project records for auditability and traceability.

Requester Details

Name, role, contact details, and submitting organization so the originator can be contacted for clarification or follow-up questions.

Description

Concise scope of the proposed change, affected components, and rationale explaining why the change is needed or requested.

Impact Assessment

Estimated schedule changes, budget adjustments, resource effects, quality implications, and any regulatory or safety considerations.

Implementation Plan

Steps, responsible parties, dates, rollback procedures, and any dependencies required to implement the change safely and efficiently.

Approvals

Designated approvers, signature blocks, dates, and conditional approvals (e.g., budget threshold or phased acceptance) recorded clearly.

Required Fields and Essential Data

Project Name: Exact project identifier
Requester: Name and role
Change Summary: Short descriptive title
Impact Summary: Time, cost, scope notes
Estimated Cost: Amount or range
Approval Signatures: Authorized approvers

Step-by-Step: Submitting an FCR

Follow these steps to prepare and route an FCR so it is actionable and auditable.

  • 01
Draft: Complete all required fields and attach supporting documents.
  • 02
Assess: Estimate schedule, cost, and risk impacts.
  • 03
Route: Send to designated approvers in the prescribed order.
  • 04
Record: Log decision, update project baseline, and archive signed FCR.

Setting Up an Online FCR Workflow

Configure your digital workflow to match existing approval rules and ensure reliable notifications and retention.

Field Configuration
Template Name Use a clear name matching contract terminology
Signer Order Sequential or parallel per governance
Authentication Email link, SMS code, or advanced auth
Notifications Set reminders and escalation rules

Common Destinations After Approval

After approval, a completed FCR must be distributed to specific parties and systems to update baselines and contracts.

  • Project Archive: Store signed FCR in central document repository.
  • Contracting: Provide copies to procurement or contracting teams.
  • Scheduling: Update project schedule and notify resource owners.
  • Finance: Submit cost adjustments to accounting or cost control.

Technical Requirements for eSubmission and eSigning

Choose a platform that supports the file types you use and the authentication level your approvals require.

  • File Formats: PDF, DOCX, XLSX supported
  • Integrations: Salesforce, NetSuite, Microsoft 365
  • Authentication: Email, SMS code, or advanced

Typical Timelines and Review Deadlines

Set clear deadlines for review and implementation so change control does not stall project momentum.

Submission Window:

Submit FCR as soon as change is identified to avoid rework.

Initial Review:

Technical review usually within 3–5 business days.

Approval Decision:

Approvers typically respond within 7–14 business days.

Implementation Start:

Begin after approvals and procurement actions complete.

Closeout:

Document completion and lessons learned within 30 days.

Key Processing Milestones

Use milestone checkpoints to monitor an FCR from submission through closeout and to trigger updates in project controls.

01

Initiation

FCR created and logged for tracking and assignment.

02

Impact Analysis

Technical, schedule, and cost impacts are estimated.

03

Approval

Authorized signers accept, reject, or conditionally approve.

04

Implementation

Work executed, baseline updated, and FCR archived.

Common Preparation Errors to Avoid

  • Incomplete impact assessments that omit schedule knock‑on effects lead to underpriced or infeasible change plans.
  • Vague descriptions lacking scope boundaries cause disputes about what work the change authorizes or excludes.
  • Missing supporting documentation, like drawings or test results, forces iterative review cycles and delays approvals.
  • Failing to route to all required approvers results in nonconforming implementations and potential contractual breach.

Risks and Consequences of Poorly Managed FCRs

Schedule Slip: Extended project timelines
Cost Overrun: Budget increases and disputes
Regulatory Noncompliance: Violations requiring remediation
Contract Dispute: Claims and litigation risk
Rejected Change: Wasted work and payments withheld
Audit Findings: Recordkeeping deficiencies cited

Real-World Examples of FCR Use

These short examples show how different organizations use an FCR template to improve clarity and speed decisions.

Optica Ventures (COO)

A mid‑size development team used an FCR to capture scope changes clearly and reduce rework.

  • The form linked approvals to budget items quickly.
  • As COO Brian Fitzgibbons noted, a simple, consistent form made it easier for customers and internal teams to understand decisions and reduced iterative clarifications during delivery.

Martin Properties (Founder)

A real estate operator standardized FCRs for retrofits to avoid schedule overruns.

  • On‑site contractors completed required fields before work began.
  • Tim Martin reported that having compliant, auditable FCR records allowed faster approvals, clearer contractor billing, and better documentation for tenants and inspectors.

Practical Tips to Improve FCR Accuracy and Speed

Adopt consistent practices that reduce rework, speed approvals, and make records defensible during audits or disputes.

Use a Controlled Template Version
Lock fields that must not change and maintain version history. This prevents unauthorized edits and improves traceability across revisions.
Attach Supporting Evidence
Include drawings, cost breakdowns, test data, and vendor quotes so reviewers can make informed decisions without follow‑up requests.
Define Approval Authority
Document who can approve cost or scope thresholds to avoid routing delays and ensure consistent governance in emergencies.
Record Conditional Approvals
When approval is contingent on conditions, document required follow‑up actions and responsible parties to prevent misunderstandings.
