What the Project Management Lead Door Document Is

The Project Management Lead Door Document is a structured project intake and authorization form used to record project scope, lead assignment, access permissions, and initial milestones for new or transitioned workstreams. It centralizes contact information, budget estimates, risk flags, and approvals required to open a new project door—ensuring a single source of truth for project initiation and handoff between stakeholders and delivery teams.

Why teams use this document

Use the Project Management Lead Door Document to standardize project intake, reduce onboarding delays, and capture approval evidence. Consistent documentation improves accountability, reduces rework, and makes resourcing and scheduling decisions auditable across the lifecycle.

Why teams use this document

Who typically completes and relies on the form

This document is completed by project sponsors, PMOs, or assigned project leads when initiating or transferring responsibility for a project.

  • Project Sponsors or Owners — Request resources, set high-level scope and budget, and provide executive approval details.
  • Project Management Office (PMO) — Standardizes intake, assigns lead, logs priority and compliance requirements.
  • Assigned Project Lead — Confirms resource availability, accepts scope, and records start milestones and dependencies.

The completed form is used by delivery teams, resource managers, procurement, and stakeholders to proceed with planning and execution.
