What the Project Management Office Move Form Is

The Project Management Office Move Form is an internal project document used to record and authorize relocation activities managed by a PMO. It captures key project identifiers, scope of the move, assets and equipment lists, assigned owners, schedule windows, site access requirements, and approver signatures. The form standardizes decision points, documents approvals, and creates an auditable record for facilities, IT, procurement, and project teams. It is suitable for office relocations, desk moves, equipment transfers, and phased site decommissioning where multiple stakeholders must confirm readiness and acceptance.

Why a Standard Move Form Matters for the PMO

A standardized move form reduces coordination gaps by centralizing approvals, clarifying responsibilities, and preserving an auditable record of decisions and asset transfers. It helps minimize downtime, align cross-functional teams, and supports compliance with internal controls and external audit requirements.

Why a Standard Move Form Matters for the PMO

Who Typically Completes and Signs This Form

The form is completed collaboratively by project, facilities, IT, and procurement leads and routed for formal approval.

  • PMO Manager or Director — Oversees move plan, approves schedule and resource allocation.
  • Facilities Manager — Confirms physical site readiness, access, and safety requirements.
  • IT Lead — Validates asset inventories, network changes, and equipment relocation procedures.

Signatures should come from authorized approvers associated with each discipline to ensure accountability and a clear escalation trail.

Stepwise Instructions to Complete and Approve the Form

Follow these steps to ensure the form is complete, routed, and retained correctly.

  • 01
Prepare form: Populate all required fields and attach inventory lists.

  • Security: TLS in transit and AES-256 at rest

Confirm BAA availability for health-related data and 21 CFR Part 11 support for regulated processes when required by policy.

Essential Sections to Include on a Professional Move Form

A complete form groups administrative, technical, and approval information into clear sections for efficient processing and auditability.

DOCX Template

Maintain an editable template to support versioning, bulk population, and localization for different sites.

CSV Inventory

Export asset lists as CSV for import into CMDB or asset management systems to reconcile inventories.

Audit Log

Export machine-readable audit reports showing timestamps, IPs, and signer actions for compliance reviews.

Security and Compliance Features to Verify

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Audit Trail: Full timestamp, IP, and action logs
ESIGN / UETA: Meets U.S. electronic signature legal standards
HIPAA Support: BAA available where PHI is present
21 CFR Part 11: Support for FDA-regulated records
Certifications: SOC 2 Type II, ISO 27001 available

Common Preparation and Execution Challenges

  • Incomplete inventory lists that omit serial numbers or asset tags lead to reconciliation delays and potential asset loss.
  • Unclear approval order causes rework when signers disagree or when parallel approvals miss required sequential checks.
  • Using inconsistent project identifiers creates duplicate records and complicates reporting across financial and PMO systems.
  • Failure to capture acceptance criteria and post-move signoffs can result in unresolved damage claims and service SLA disputes.

Operational and Compliance Risks of Incorrect Forms

Operational Delay: Missed move windows and vendor penalties
Asset Loss: Untracked equipment increases loss risk
Contractual Exposure: Unapproved changes breach vendor agreements
Regulatory Risk: Improper PHI handling may trigger HIPAA issues
Audit Findings: Incomplete records can cause negative audit outcomes
Financial Penalty: Unexpected remediation costs and fines
