This Prom Agreement is entered into by Student and Parent/Guardian listed above for attendance at the event described. Student and Parent/Guardian acknowledge and agree that attendance is conditional upon compliance with the obligations, conditions and rules set forth in this document and any applicable school or venue policies.

Student Obligations and Code of Conduct

The Student agrees to the following prior to and during prom and related activities:

Parent/Guardian agrees to be financially responsible for damage to property, unauthorized expenses, or costs incurred due to Student's misconduct related to the event.

In the event of a medical emergency and where the Parent/Guardian cannot be reached, Parent/Guardian authorizes designated chaperones or medical personnel to provide or seek emergency medical treatment for the Student, including transportation and necessary medical interventions.

Liability, Release and Indemnity

The Parent/Guardian and Student acknowledge that attendance carries inherent risks. By signing, Parent/Guardian releases and holds harmless the school, venue, chaperones, volunteers, and their respective agents from liability for personal injury, property damage, or other loss that may arise from Student's attendance, except to the extent caused by gross negligence or willful misconduct of the released parties. Parent/Guardian agrees to indemnify and defend the released parties against claims arising from Student's acts or omissions.

Photographs and Media

I grant permission for photographs and video including the Student to be used by the school or event organizers for promotional or archival purposes.
I do not grant permission for photographs or video of the Student to be used.

Breach, Remedies and Disciplinary Action

If the Student violates any provision of this Agreement, the Student may be removed from the event, required to leave immediately at Parent/Guardian expense, and subject to school disciplinary procedures. Parent/Guardian acknowledges that the school may contact law enforcement for suspected criminal conduct.
