What the Property Appraisal Request Form Is

The Property Appraisal Request Form is a standardized document used to instruct a licensed appraiser or appraisal management company to perform a valuation of real property. It identifies the property, the requester, the scope of work (type of value opinion, intended use, effective date), access and contact details, and any required attachments such as prior appraisals, floor plans, or survey data. The form creates an audit trail of the request and helps ensure the appraisal meets the client’s regulatory, underwriting, or tax reporting needs while documenting delivery expectations and fee arrangements.

Why a Clear Request Form Matters

A complete appraisal request reduces scope misunderstandings, improves turnaround time, and helps the appraiser deliver a report that meets lender, insurer, or internal compliance requirements.

Why a Clear Request Form Matters

Who Commonly Prepares or Signs the Form

Each signer or preparer should confirm role, authority to request the appraisal, and any payer or billing arrangements before submission.

  • Lenders and underwriters requesting valuations for mortgage approval, portfolio review, or collateral monitoring.
  • Property owners or asset managers ordering appraisals for sale, insurance, or tax appeals.
  • Real estate brokers, attorneys, and tax advisors coordinating valuation work for transactions or disputes.

Step-by-step: Completing a Property Appraisal Request Form

Follow these steps to ensure the request is actionable and compliance-ready.

  • 01
Identify Parties: Enter requester, client, and appraiser contact details clearly.
  • 02
Describe Property: Provide address, legal description, and parcel or tax ID.
  • 03
Define Scope: Select type of appraisal, intended user, and effective date.
  • 04
Attach Documents: Include plats, surveys, leases, prior reports, and photos.

Key Sections to Include in a Professional Request

A professional appraisal request groups administrative, property, scope, access, and legal terms in clear sections to reduce ambiguity.

Administrative

Requester contact, billing instructions, and preferred delivery method. Clear admin details reduce routing delays and ensure timely payment.

Property Details

Complete address, legal description, parcel ID, zoning, and measured area. Accurate property details are essential for correct identification and comparable selection.

Scope of Work

Specify report type (full appraisal, drive-by, desktop), intended use, and whether an appraisal review is required; define any limiting conditions.

Attachments

Include surveys, site plans, leases, rent rolls, prior appraisals, and photos. Attachments provide context and reduce follow-up questions.

Access & Timing

Note access instructions, occupant contact, and requested delivery deadline. Provide flexibility windows for inspections when possible.

Legal & Compliance

State governing law, confidentiality terms, and fee/engagement clauses. Include HIPAA addenda for health-care related property if PHI is present.

Essential Data Elements to Protect

PII: Names and SSNs
Property IDs: Parcel and tax identifiers
Contact Info: Emails and phone numbers
Legal Docs: Leases and contracts
Photos: Site and interior images
Payment Data: Billing and card info

Typical Request-to-Report Workflow

A clear digital workflow shortens cycle time and preserves an audit trail for compliance and underwriting.

  • Request Submission: Submit form with attachments to selected appraiser or AMC.
  • Order Acceptance: Appraiser confirms scope, fee, and inspection window.
  • Inspection: On-site or remote inspection completed per agreed scope.
  • Report Delivery: Final report delivered with completion certificate and audit trail.

How to Configure an Electronic Request Workflow

Configure fields and routing so the request, authorization, inspection notes, and final report move through a single, auditable process.

Field Configuration
Requester Required field, contact and billing info
Appraiser Selector for licensed appraiser or AMC
Attachments Accept PDF, DOCX, JPG; max 25 MB
Routing Auto-notify appraiser, then client on completion

Digital Delivery and Platform Considerations

Ensure the platform can produce a tamper-evident signed PDF and retain an audit trail consistent with ESIGN/UETA requirements.

  • Formats: PDF and Word (DOCX) are standard for signed reports.
  • Integrations: Connectors: Salesforce, NetSuite, Microsoft 365, Google Workspace.
  • Security: TLS 1.2/1.3 and AES-256 encryption at rest.

Typical Timelines and Delivery Expectations

Turnaround time depends on scope, property type, and access; set realistic deadlines and note escalation steps for urgent orders.

Standard Turnaround:

7–14 business days for full appraisals

Drive-by/Desktop:

2–5 business days if data suffices

Rush Orders:

24–72 hours with additional fee

Inspection Window:

Coordinate 2–3 available dates for access

Extension Requests:

Appraiser should notify requester in writing

Key Milestones from Order to Report

A numbered milestone sequence clarifies responsibilities and expected timing for each step.

01

Order Placed

Requester submits completed form and attachments.

02

Order Accepted

Appraiser confirms scope and inspection date.

03

Inspection Complete

On-site or remote inspection performed and photos taken.

04

Report Issued

Final report delivered and uploaded to requester portal.

Common Mistakes That Delay Appraisals

  • Incomplete address or incorrect parcel ID that prevents locating the subject property and delays scheduling.
  • Missing attachments such as leases, prior appraisals, or floor plans that force scope clarifications and fee adjustments.
  • Unclear access instructions leading to missed inspections or the need for second visits and added costs.
  • Vague intended use statements that produce a report not suited for the client’s regulatory or underwriting needs.

Risks of an Incorrect or Incomplete Request

Delayed Funding: Loan closings postponed
Additional Fees: Re-inspection or scope change charges
Noncompliance: Failure to meet USPAP or lender standards
Invalid Report: Report not accepted by intended user
Privacy Breach: Improper handling of PII or PHI
Liability: Potential professional disputes
