What a Provider Charter Template Is and When it Applies

The Provider Charter Template is a standardized document that records expectations, responsibilities, and governance controls between an organization and a service provider. It typically defines the scope of services, measurable service levels, reporting cadence, escalation paths, data handling rules, performance metrics, and termination conditions. Organizations use the template to align legal, procurement, and operational teams on deliverables and compliance obligations. Where permitted by law, the charter can be executed electronically under the ESIGN Act (15 U.S.C. §7001) and applicable state UETA statutes to create an enforceable record.

Why a Provider Charter Template Matters to Your Vendor Program

A Provider Charter Template reduces ambiguity, helps prevent disputes, and creates a single source of truth for vendor obligations. It supports measurable governance, streamlines onboarding, and aids audit readiness by documenting service levels, reporting requirements, and compliance responsibilities in one place.

Why a Provider Charter Template Matters to Your Vendor Program

Who Typically Completes and Relies on This Template

Organizations across procurement, legal, compliance, and operations teams use the Provider Charter Template to align internal and external expectations.

  • Procurement and vendor managers who establish contract deliverables and performance monitoring responsibilities.
  • Compliance and privacy officers who verify contractual controls, data protection terms, and regulatory alignment.
  • Service providers and vendor operations teams that must accept reporting, escalation, and service level obligations.

Vendors and their account or program managers also use the charter to confirm responsibilities and provide the periodic reports required by the charter.

Core Sections to Include in a Professional Provider Charter
