What the Public Relations Request Form Is

A Public Relations Request Form is a standardized internal document teams use to request PR support, media outreach, or publicity assets from a communications function. It captures the campaign overview, objectives, target audiences and outlets, key messages, assets to be supplied, timing (including embargo dates), approvals required, and point(s) of contact. The form creates a single source of truth for PR intake, reduces back-and-forth clarification, and ensures legal, compliance, and media-relations checks are completed before distribution. It is used by marketing, product, legal, and executive stakeholders.

Why a Standard Request Form Helps Your Team

A consistent Public Relations Request Form improves clarity, shortens turnaround, and documents approvals and embargoes to reduce legal and reputational risk. It ensures required reviews (brand, legal, regulatory) occur before outreach and tracks attachments, spokesperson availability, and distribution channels.
