RECITALS

The Plan Administrator shall determine the form and amount of benefits in accordance with Plan provisions and may request judicial clarification if necessary.

7. ENFORCEMENT; COURT JURISDICTION

The Court retains jurisdiction to enforce this Order and to amend or clarify this Order to the extent necessary to effectuate its intent and to satisfy applicable law and Plan requirements. The parties shall cooperate in providing any additional information reasonably required by the Plan Administrator to implement this Order.

8. NOTICES

Any notices required by this Order shall be delivered to the addresses below by certified mail, hand delivery, or other method providing written confirmation of receipt.

9. GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY

This Order shall be governed by and construed in accordance with the laws of the State of . This Order contains the entire understanding of the parties and supersedes any prior orders or agreements that are inconsistent with its terms. If any provision of this Order is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.
