While the Lease was in effect, (Name) was the (Operator/Owner) of the Lease. There were ____ wells drilled on the Lease, which had production equipment attached or connected to them. The wells ceased to produce (oil/gas) in (month/year). The Lea se is deemed to have (expired/terminated) due to the lack of production from the wells and no other action being taken to maintain the Lease in force and effect.
