What the Radon Testing Form Is and when it applies

A Radon Testing Form documents the objective collection, chain of custody, and results of radon measurements performed at a property. It records test method, device ID, placement location, start and end times, and laboratory analysis or detector readout. The form supports real estate disclosures, inspection reports, mitigation planning, and regulatory records. Proper completion preserves evidentiary value, helps manage buyer and seller expectations, and creates a clear record for follow-up testing or mitigation if elevated radon levels are detected.

Why a clear Radon Testing Form matters

A consistent, complete Radon Testing Form reduces disputes, streamlines real estate transactions, and preserves the integrity of test data. It documents placement and timing that affect results, protects parties by showing chain of custody, and supports mitigation decisions when levels exceed EPA action guidelines.

Why a clear Radon Testing Form matters

Who typically completes or relies on this form

Primary users range from testing technicians to real estate professionals; others rely on the results for safety and disclosure.

  • Licensed Radon Tester — Performs the measurement, records device ID and placement, and certifies test conditions.
  • Real Estate Agent — Uses results for property disclosures and to advise buyers or sellers during escrow periods.
  • Homebuyer / Seller — Reviews test outcomes to evaluate health risk, negotiating repairs or mitigation as needed.

The completed form becomes part of due diligence files, buyer disclosures, inspection reports, and mitigation project records.

Core components of a professional Radon Testing Form

A robust form groups identification, test method, placement details, timing and environmental conditions, results with units, and chain-of-custody or certification statements to ensure defensibility and clarity for all parties.

Property ID

Street address, unit number, city, state, ZIP; contact name and phone for site access and record matching.

Test Method

Type of test (short-term charcoal, alpha-track, continuous monitor), device model and serial number, and laboratory or manufacturer.

Placement Details

Exact room and position (e.g., lowest livable level, 3 ft from exterior wall), height above floor, and avoidance of drafts or windows.

Timing & Conditions

Start and end date/time (MM/DD/YYYY HH:MM), closed-house conditions, HVAC status, and notable openings during test.

Results Summary

Measured radon concentration (pCi/L), measurement uncertainty if provided, and lab-certified interpretation.

Chain of Custody

Signature and printed name of tester, date signed, and space for lab receipt or electronic verification details.

Essential data fields to include

Device ID: Serial or lot number
Start Date: MM/DD/YYYY format
End Date: MM/DD/YYYY format
Test Type: Short-term or long-term
Reading Value: Numeric pCi/L result
Tester Name: Printed name and signature

Step-by-step: Completing a Radon Testing Form

Follow a consistent workflow to collect valid data and produce a defensible record for disclosure or mitigation planning.

  • 01
Schedule Test: Confirm access, closed-house window, and device availability before arrival.
  • 02
Prepare Site: Place device per protocol and record exact placement and environmental conditions.
  • 03
Run Test: Start device, observe required closed-house period, and log start/end timestamps.
  • 04
Document & Submit: Record readings, sign chain-of-custody, and deliver results to client or listing agent.

Customizing an online Radon Testing Form workflow

Configuring the digital workflow ensures accurate data capture, signer authentication, and consistent delivery to stakeholders.

Field Configuration
Upload Format Accept PDF, DOCX; preserve layout during import
Auto Fields Enable automatic detection for address and dates
Authentication Use email or SMS code for signer verification
Delivery Send signed copy to tester, agent, and client automatically

Where to send the completed Radon Testing Form

Route the completed form to all parties who rely on results, and retain a copy for records and potential audits.

  • Client Copy: Provide the buyer or seller a signed copy for disclosure files.
  • Listing Agent: Send to the agent to include in the transaction package.
  • Lab / Certifier: Attach device data or lab report for verification if applicable.
  • File Retention: Store a copy in transaction records and tester files for the retention period.

Sharing and eSubmission options for the form

Digital submission supports signed PDFs, secure email, and integration into transaction management systems.

  • File Formats: PDF/A and DOCX supported
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Authentication: Email or SMS code verification

Typical timelines and result turnaround to plan for

Track test duration, lab turnaround, and transaction inspection periods so results meet disclosure deadlines and mitigation planning windows.

Short-term Test:

48–96 hours typical; follow device manufacturer protocol.

Long-term Test:

90+ days for seasonal average and lower variability.

Lab Analysis:

Laboratory turnaround commonly 3–7 business days after receipt.

Inspection Window:

Align testing to fall within contract inspection contingency periods.

Mitigation Start:

Schedule mitigation after confirmed elevated results and agreed repair terms.

Common mistakes to avoid when preparing the form

  • Placing the detector near doors or windows alters airflow and can invalidate results if closed-house conditions are not met.
  • Failing to record exact start and end timestamps undermines chain of custody and can cause disputes in transaction negotiations.
  • Using the wrong test type for the objective (short-term when a long-term average is needed) yields unrepresentative exposure estimates.
  • Not retaining or delivering the signed form to all transaction parties delays disclosure and may impede closing or mitigation scheduling.

Risks and legal consequences of incomplete or incorrect forms

Transaction Delay: Escrow holds or renegotiation
Buyer Rescission: Contract may be voided
Liability Claims: Claims for nondisclosure or negligence
Invalid Results: Test may be rejected by buyer or lab
Cost Overruns: Additional testing and mitigation expenses
Record Gaps: Unable to defend test accuracy

Export and storage formats for signed testing records

Choose formats that preserve signatures, metadata, and audit trails. Export options influence long-term accessibility and legal defensibility.

Portable PDF

Export as PDF/A to preserve visual layout, embedded signature, and metadata for long-term archival and audit purposes.

Native DOCX

Keep an editable DOCX for internal templates and future form revisions, but preserve a signed PDF for records.

CSV Data Export

Export tabular fields (addresses, dates, numeric readings) as CSV for integrations with MLS or reporting systems.

Audit Report

Include an audit trail file showing signer attribution, IP address, timestamp, and actions for evidentiary support.

Practical examples of how the form is used

Two typical scenarios illustrate how a completed Radon Testing Form supports decision-making and transaction flow.

Resale Transaction

Seller orders a short-term test during the inspection period and records exact placement and timestamps.

  • The buyer reviews the certified PDF and requests mitigation when levels exceed 4.0 pCi/L.
  • The signed form and lab report are attached to the closing packet to document disclosure and to schedule mitigation before closing.

Post-Mitigation Verification

Contractor performs follow-up testing after installation of a mitigation system and documents device location and readings.

  • Results certify system performance and inform warranty timelines.
  • The form is filed with the contractor record, provided to the homeowner, and retained for seven years to support future resale disclosure.
