What the Real Estate Vendor Questionnaire Is and When it’s Used

A Real Estate Vendor Questionnaire is a standardized form used by property owners, managers, brokers, and procurement teams to collect identity, compliance, insurance, licensing, and payment details from third‑party vendors. It centralizes verification items such as legal entity name, tax identification, insurance certificates, business licenses, scope of services, and references. The questionnaire supports onboarding, risk assessment, and 1099 reporting preparation and is often required before a vendor is added to accounts payable or allowed on site.

Why a Formal Questionnaire Matters for Real Estate Operations

A consistent vendor questionnaire reduces onboarding friction, documents regulatory and insurance compliance, and creates an auditable record for tax reporting and vendor risk management.

Why a Formal Questionnaire Matters for Real Estate Operations

Typical Roles That Complete or Request the Questionnaire

The form is shared and completed by a mix of internal teams and external vendors during onboarding or contract setup.

  • Property managers and asset managers who vet vendors for insurance and performance requirements.
  • Procurement and accounts payable teams who need tax and payment setup details for 1099 reporting.
  • Vendors and subcontractors who supply certificates of insurance, licenses, and W‑9 information.

Use role-specific routing and required‑field checks to ensure the right stakeholders review and approve vendor submissions.

Who Signs and Submits the Questionnaire

Property Manager

Typically initiates the request and reviews responses. Responsible for verifying insurance limits, scope alignment, and that licensing matches the service being performed before approving site access or payment.

Vendor Representative

An owner, officer, or authorized agent of the vendor completes and signs the questionnaire, certifying accuracy of tax ID, insurance, licenses, and contact details for accounts payable.

Core Sections to Include in a Professional Questionnaire

A comprehensive questionnaire structures vendor data for verification, compliance, and recordkeeping to minimize onboarding delays and support audit trails.

Vendor Identity

Legal business name, DBA, entity type, and Employer Identification Number (EIN) to match tax and payment records and avoid backup withholding issues.

Contact Details

Primary contact name, email, phone, and remit address so purchase orders, invoices, and tax documents route correctly to accounts payable.

Insurance Certificates

Insurance carrier names, policy numbers, coverage limits, and certificate holder language required by the property owner to manage liability exposure.

Licenses & Permits

State and local license numbers, issue/expiration dates, and jurisdiction to confirm contractor eligibility for regulated work at the property.

Service Scope

Clear description of services, locations, work hours, and any subcontracting so risk and access controls can be applied consistently.

Tax & Payment Setup

W‑9 or W‑8, preferred payment method, and ACH details to complete vendor setup and support 1099 reporting compliance.

Security, Compliance, and Technical Controls to Record

Encryption: TLS 1.2/1.3 in transit, AES‑256 at rest
Audit Trail: Timestamps, IP addresses, and action history
Certifications: SOC 2 Type II, ISO 27001
Privacy Compliance: HIPAA BAA available when needed
eSignature Law: ESIGN and UETA compliant
Access Controls: Role‑based permissions and SSO

Key Risks and Regulatory Penalties to Watch For

Incorrect TIN: Triggers 24% backup withholding
Late 1099 Filing: $60–$330 per form (IRC §6721)
Intentional Disregard: $660+ per form, no cap
Missing Insurance: Increases liability exposure
Unauthorized Signatory: May void contractual obligations
Weak Records: Complicates audits and litigation

Common Preparation Errors That Delay Onboarding

  • Incomplete W‑9s or mismatched legal names that prevent accounts payable from validating the vendor for 1099 reporting.
  • Expired or missing insurance certificates that fail to list the property owner as certificate holder or include required additional insured language.
  • Unclear service descriptions that omit locations or subcontracting details, creating scope and liability ambiguity.
  • Incorrect or absent contact and remit information that delays invoice processing and creates duplicate vendor records.

Step-by-Step: How to Complete the Questionnaire

Follow these steps to collect, verify, and finalize vendor information efficiently and maintain an auditable record.

  • 01
Request Form: Send the questionnaire and required attachments to vendor.
  • 02
Vendor Completes: Vendor fills fields and attaches W‑9, insurance, licenses.
  • 03
Internal Review: Verify insurance, licenses, and TIN against records.
  • 04
Finalize Onboarding: Approve vendor, create payment record, and store documents.

Recommended Digital Workflow Settings for Online Completion

Configure the electronic workflow to require attachments, enforce required fields, and route completed questionnaires for internal approval.

Field Configuration
Required Attachments W‑9, insurance, licenses
Field Validation TIN format checks, date pickers
Signer Authentication Email link with optional SMS code
Approval Routing Property manager → procurement → AP

Typical Submission and Verification Flow

A clear end‑to‑end flow reduces manual checks and creates a tamper‑evident audit trail for vendor onboarding.

  • Upload: Sender places fields and attachments on the questionnaire.
  • Send: Vendor receives secure signing link or email invitation.
  • Verify: Reviewer checks insurance, licenses, and TIN accuracy.
  • Store: Finalize onboarding and save documents to records.

Technical and Integration Considerations for eSubmission

Choose a platform that supports secure file formats, integrations with accounting systems, and audit trails for legal defensibility.

  • File Formats: PDF, DOCX, and scanned images supported
  • Integrations: Salesforce, NetSuite, Microsoft 365, Google Workspace
  • Access Controls: SSO, role permissions, and audit logs

Ensure the platform can produce a tamper‑evident certificate of completion and retains records in compliance with applicable statutes and internal retention policies.

Timeframes and Deadlines to Plan For

Track required turnaround times for onboarding tasks and statutory filing deadlines to avoid operational or regulatory penalties.

W‑9 Submission:

No statutory deadline; provide upon request to avoid backup withholding.

1099 Reporting:

Annual recipient/IRS deadlines: Jan 31 for 1099‑NEC and many recipient copies.

Vendor Onboarding:

Typical verification completes within 3–10 business days depending on document completeness.

Insurance Renewal:

Require renewal certificate prior to policy expiration to maintain site eligibility.

Retain Originals:

Keep records according to retention policies tied to tax and liability statutes.

Key Onboarding Milestones and Where Delays Occur

Use a milestone checklist to identify bottlenecks from request to active vendor status.

01

Request Sent

Vendor receives questionnaire and required document list.

02

Documents Returned

Vendor uploads W‑9, insurance, and licenses; missing items cause the biggest delays.

03

Internal review confirms coverage, license validity, and TIN match.

04

Active Vendor

Accounts payable creates vendor record and schedules first payment.
