What the Recognition High Five Document Is

The Recognition High Five Document is a concise, standardized record used to acknowledge and document employee recognition, peer-to-peer praise, or short-form awards. It captures who is being recognized, who nominated them, the reason for recognition, date, and approver details. Organizations use it to create an auditable trail for internal culture programs, track informal rewards, and surface examples for performance reviews. When executed electronically, the document can include an audit trail showing intent and attribution consistent with ESIGN and applicable state UETA laws for interstate and intrastate transactions.

Why a Formal Recognition Record Matters

A single standardized form preserves the facts around recognition, reduces disputes, and supports HR reporting and inclusion in performance records.

Why a Formal Recognition Record Matters

Who Typically Creates and Uses This Document

The Recognition High Five Document is used across HR, people managers, and individual contributors to record positive actions and informal awards.

  • HR administrators who manage recognition programs and maintain records for reporting and retention.
  • Managers who nominate or approve recognition to document performance examples and justify discretionary awards.
  • Employees who submit peer recognition entries that may feed team spot-award processes.

Use it as a lightweight workflow to route approvals, centralize recognition data, and feed performance discussions or reward disbursement.

Typical Signers and Roles

HR Administrator

HR administrators configure the form, set approval flows, and retain the completed records for compliance, payroll, and internal audit. They ensure fields capture necessary tax or award details and that retention meets company policy and legal requirements.

Download, Attachments, and Audit Features

Download Formats

Save completed records as PDF or DOCX to preserve layout and metadata; PDFs are preferable for long-term archiving and tamper evidence.

Attachments

Attach supporting materials such as photos, project summaries, or manager notes. Label attachments clearly and maintain version control for context.

Audit Trail

Access Controls

Restrict view and edit rights to HR and designated approvers to protect privacy and prevent unauthorized changes.

Practical Tips for Reliable Recognition Records

Adopt consistent templates and clear routing to reduce errors and ensure the recognition data supports HR metrics.

Use Standardized Language
Adopt controlled wording for reasons and categories so analytics and performance systems can classify entries consistently without manual rework.
Require Minimal Mandatory Fields
Limit required fields to essentials (name, date, reason, approver) to reduce friction while ensuring adequate detail for audit and reporting.
Preserve an Immutable Record
Lock finalized entries and retain the audit trail. If edits are necessary, record an amendment entry rather than overwriting originals.
Integrate with HR Systems
Automate employee lookup and approval routing by integrating with HRIS to prevent data entry mistakes and improve reporting accuracy.

Timelines and Routine Reporting Cadence

Define clear internal deadlines to ensure recognition is timely and properly recorded for each pay or review cycle.

Immediate Submission:

Submit within 7 days of the recognized action.

Monthly Review:

HR reconciles entries at month end for reporting.

Quarterly Reporting:

Aggregate recognition data for performance reviews quarterly.

Tax Year Close:

Confirm taxable award records before year-end payroll closing.

Retention Check:

Verify archival tags at annual compliance review.
