Assumption of Risk

I acknowledge that participation in recreational pool table activities involves inherent risks, including but not limited to slips, trips, falls, collisions, impact from cues or balls, musculoskeletal injuries, and property damage. I freely choose to participate despite such risks and accept full responsibility for my actions and any resulting injury or damage.

I have read and understand the Assumption of Risk clause and voluntarily accept those risks.

Release and Waiver of Liability

In consideration of being permitted to use Provider's facilities and equipment, I, on behalf of myself, my heirs, assigns, personal representatives and next of kin, hereby release, waive, discharge, and covenant not to sue Provider, its owners, agents, employees, and volunteers (collectively, Released Parties) from any and all liability, claims, demands, causes of action or damages for personal injury, property damage, wrongful death, or any other loss arising out of or related to my use of the pool table facilities, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

Indemnification

I agree to indemnify, defend and hold harmless the Released Parties from and against all claims, demands, liabilities, damages, losses, costs and expenses (including reasonable attorneys' fees) arising out of or resulting from my negligent acts or omissions or willful misconduct while using Provider's equipment or premises.

Medical Authorization

In the event of an injury or medical emergency, I authorize Provider and its representatives to provide or obtain emergency medical treatment for me and to make related decisions for the purpose of preserving my life or health. I agree to be financially responsible for any medical treatment rendered.

Damage to Equipment and Fees

Participant agrees to exercise reasonable care while using cues, balls, and tables. Participant shall be responsible for repair or replacement costs for damage caused by Participant's negligence or intentional acts. Provider may assess fees for repair or replacement and may withhold access until payment is made.

Photo and Video Release

I consent to the use of photographs and video recordings that may be taken of me while participating in activities at the facility for promotional, educational, or other lawful purposes without compensation. If I do not consent, I will notify Provider in writing prior to participation.

If Participant is under 18 years of age, a parent or legal guardian must complete this section and sign on behalf of the minor. By signing, the parent/guardian accepts the terms of this Waiver for the minor and represents that they have the legal authority to do so.
