What the ReCred Document Is and When it Applies

The ReCred Document is a provider re-credentialing application used by healthcare professionals and organizations to renew network participation, verify current licensure, insurance, privileging, and professional history, and to attest to continuing qualifications. Typical users include clinicians, credentialing coordinators, managed care organizations, and payer network administrators. The form collects identifying information, education and training, malpractice history, hospital privileges, board certifications, and attestation statements required by payers and credentialing committees to meet regulatory and contractual standards.

Why a Complete ReCred Document Matters

Accurate re-credentialing protects patient access, maintains provider network status, supports timely claims processing, and creates an auditable record for compliance with payer contracts and regulatory oversight.

Why a Complete ReCred Document Matters

Primary Users and Stakeholders

The ReCred Document is completed and managed by several role types across clinical and administrative teams.

  • Healthcare providers and clinicians responsible for keeping their credentials current and attesting to professional status.
  • Credentialing specialists and practice managers who prepare, review, and submit documentation to payers and privileging committees.
  • Payer network administrators and medical directors who evaluate eligibility, approve re-credentialing, and maintain network rosters.

Core Sections a Professional ReCred Document Should Include

A complete ReCred Document groups information logically to support verification. The following six components are typically required and should be organized for reviewer efficiency and audit readiness.

Provider ID

Full legal name, NPI, tax identification (TIN/EIN), date of birth, and primary practice location. Matching these fields to payer records avoids TIN or NPI mismatches that can trigger denials and backup withholding.

Licenses

State medical or professional license numbers, issuing board, expiration dates, and any active or past disciplinary actions. Include license lookup URLs or printouts when available to speed verification.

Education

Medical school, residency, fellowship, board certifications, and completion dates. Attach certificates and primary source verifications where required by payers or privileging bodies.

Malpractice

Claims history, current malpractice insurer name, policy limits, policy period, and copies of declarations pages. Disclose pending or settled matters as required by payer questionnaires.

Privileges

Hospital privileges and membership status including facility names, service dates, and any restrictions or suspensions. Attach privilege letters when used for payer credentialing.

Attestation

Signed attestations covering accuracy of information, consent to primary source verification, and acknowledgment of reporting obligations. Include signature date and signatory authority details.

Step-by-Step: Completing and Submitting the ReCred Document

Follow these sequential steps to prepare and submit a complete application that meets payer expectations.

  • 01
Gather records: Collect licenses, certificates, privilege letters, and malpractice declarations.
  • 02
Complete form: Populate all fields accurately, using MM/DD/YYYY for dates.
  • 03
Attach documents: Upload scanned or native PDFs for primary source documents.
  • 04
Submit and track: Send to payer or credentialing committee and retain the audit trail.

Typical eSubmission Workflow for the ReCred Document

Electronic submission streamlines verification and creates an auditable record; the workflow below outlines a common routing pattern.

  • Upload: Sender uploads completed form and supporting files.
  • Place fields: Designate signature, date, and attachment placeholders.
  • Assign reviewers: Set routing order for internal credentialing staff and external payers.
  • Capture audit: System records timestamps, IPs, and signer authentication events.

Configuring an Online ReCred Workflow

Key settings reduce friction and help ensure secure, auditable submissions when configuring an electronic workflow.

Field Configuration
Authentication Email + SMS code or higher for payer-required identity proofing
Field Types Signature, initials, date, file attachment, conditional fields
Routing Order Sequential for internal review then payer acceptance
Audit Trail Enable detailed logs capturing timestamp, IP, and actions

Technical and Integration Considerations

Ensure the platform you use supports required file formats, integrations, and security controls needed for re-credentialing.

  • Formats: PDF, DOCX supported
  • Integrations: Connect with EHRs, NetSuite, Salesforce
  • Security: TLS in transit; AES-256 at rest

Typical Deadlines and Processing Expectations

Understanding common timelines helps avoid lapses in network participation; actual deadlines depend on payer contract terms and credentialing cycles.

Re-credentialing cycle:

Often every 24 months; confirm with each payer

Submission window:

Submit within the payer-specified renewal period to prevent coverage gaps

Primary source checks:

Verification may take 2–6 weeks depending on institutions

Payer decision:

Credentialing committees typically finalize within 30–90 days

Follow-up requests:

Respond promptly; delays extend processing time

Consequences of Incomplete or Incorrect ReCred Documents

Network removal: Loss of payer participation
Claim denials: Payment delays or refusals
Sanctions risk: Potential disciplinary action by payers or boards
Contract breach: Exposes practice to contractual penalties
Backup withholding: TIN mismatch may trigger withholding
Audit exposure: Insufficient records increase audit vulnerability

Common Preparation Mistakes to Avoid

  • Missing or expired licenses and certificates submitted without verification cause primary source check failures and extended processing times.
  • Inaccurate NPI or TIN entries lead to claims mismatches and potential backup withholding by payers.
  • Incomplete malpractice history, such as failing to disclose settled claims, can lead to application denials or later contract rescission.
  • Using inconsistent names (legal name vs. professional/maiden names) across documents creates identity-matching errors during verification.
