Release and Indemnity: Upon execution of a release in accordance with the terms specified in this form and any agreed conditions, Requestor shall release and forever discharge Release Authority and its affiliates from all claims, demands, causes of action, liabilities and obligations arising from the released subject matter to the extent described in the executed release instrument. Requestor agrees to indemnify Release Authority for claims arising from inaccurate representations in this form.

GOVERNING LAW

This Release Request shall be governed by and construed in accordance with the laws of the state of without regard to its conflict of law principles. Venue for any disputes arising under this form shall lie exclusively in the courts of that state unless the parties agree otherwise in writing.

ENTIRE AGREEMENT

This Release Request, together with any attachments and any subsequent written release instrument executed by the parties, constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior negotiations and understandings. Any amendment must be in writing and signed by both parties.
