What the Relocation Information Form Is

A Relocation Information Form collects the practical and administrative details needed when an employee, tenant, student, or other party moves for work, school, or program purposes. It typically captures personal contact data, proposed move dates, destination address, reimbursement preferences, employer or program approvals, and required supporting documents. Organizations use this form to coordinate logistics, calculate taxable reimbursements, and document approvals. The form can be paper or electronic; when executed electronically it must meet ESIGN and UETA requirements for signature intent, consent, attribution, and record retention.

Why a Clear Relocation Information Form Matters

A well-structured form reduces processing delays, lowers error rates, and documents approvals for tax and audit purposes. It centralizes required data so payroll, HR, and finance teams can act consistently while preserving a defensible record.

Why a Clear Relocation Information Form Matters

Who Typically Completes and Reviews This Form

Different teams interact with the Relocation Information Form depending on the move type and organizational structure.

  • Employees and transferees who provide personal details, move dates, and supporting receipts for reimbursement and tax reporting.
  • Human Resources or Mobility teams that review eligibility, approve allowances, and record relocation terms in personnel files.
  • Finance or Payroll who calculate taxable benefits, process reimbursements, and retain records for IRS or audit purposes.

Use clear role assignments to avoid duplicate submissions and to ensure each department has the data it needs for compliance and payment.

Step-by-Step: How to Complete the Form

Complete fields in the order below and confirm attachments before submission.

  • 01
Enter Personal Details: Fill name, employee ID, and contact information accurately.
  • 02
Provide Move Details: Enter move date, new address, and reason for relocation.
  • 03
Attach Proofs: Upload receipts, leases, or closing statements as required.
  • 04
Sign and Submit: Apply your signature and submit to the designated approver or system.

Typical Routing and Processing Flow

Most organizations route the completed form through a defined approval chain with automated notifications.

  • Submission: Employee uploads form and attachments to HR or system.
  • HR Review: HR verifies eligibility and completeness.
  • Finance Review: Payroll validates tax treatment and payment method.
  • Final Approval: Authorized manager signs and the file is archived.

Technology and File Format Considerations

Choose platforms and formats that preserve data integrity, secure PII, and create an auditable trail for signatures.

  • File Formats: PDF or Word DOCX recommended for compatibility and preservation.
  • Authentication: Email or SMS codes; stronger methods for high-risk moves.
  • Integrations: Connect with HRIS, payroll, and document storage systems.

Verify chosen tools support secure storage (AES-256), TLS in transit, and produce an audit trail that meets ESIGN/UETA recordkeeping expectations.

Field Configuration

Practical Tips for Accurate and Efficient Completion

Use Standardized Templates
Standardize fields and dropdowns to reduce manual entry errors; validate required fields before submission to prevent incomplete records.
Require Digital Attachments
Mandate PDFs for receipts and contracts; ensure file names include dates and document type for easier retrieval.
Document Approvals
Capture role-based approvals with timestamps to clarify authority and speed audits or disputes resolution.
Protect PII
Limit access to form data, use encryption at rest and in transit, and apply role-based permissions for viewing sensitive fields.

Common Deadlines and Processing Expectations

Timelines depend on internal policy; federal reporting and tax deadlines also apply for related documents and withholding.

Form Submission Window:

Submit relocation info as soon as move is authorized

Reimbursement Claims:

Follow employer policy for claim windows, typically 30–90 days

W-9 Requests:

Provide W-9 upon payer request; no fixed IRS deadline

W-2 Reporting:

Employers issue W-2 by Jan 31 for prior year

Tax Return:

Individual returns due April 15 (Form 1040)

Common Preparation Mistakes to Avoid

  • Incomplete or inconsistent names between payroll and identification documents cause verification failures and payment delays.
  • Missing or poor-quality attachments (illegible receipts, partial invoices) lead to rejected reimbursements and audit queries.
  • Failure to indicate taxable versus nontaxable items causes incorrect withholding and potential IRS penalties.
  • Using unsecured email or public storage for PII increases the risk of privacy breaches and regulatory exposure.
