NOW, THEREFORE, Accrediting Organization hereby certifies that Accredited Entity meets the minimum requirements for remote work accreditation for the scope described below and is authorized to undertake remote work in accordance with the terms of this Certificate. This Certificate is effective as of , unless earlier suspended or revoked in accordance with the Term and Termination provisions below.

Such audits will be scheduled with reasonable notice and conducted during normal business hours unless an urgent compliance concern exists.

In the event of noncompliance, Accrediting Organization may require remediation, impose additional controls, suspend accreditation, or revoke accreditation where necessary. Remedies provided herein are cumulative and do not exclude any remedy available at law or equity.

Governing Law

This Certificate and any dispute arising out of or relating to it shall be governed by and construed in accordance with the laws of , without regard to conflict of laws principles.

Entire Agreement
