What a Remote Work Review Form Is and When it’s Used

A Remote Work Review Form documents an employee’s request to work remotely or an employer’s periodic assessment of an existing remote arrangement. It records employee details, duties, equipment and security requirements, manager evaluation, and approval decisions. The form supports consistent decision-making, documents accommodation or operational changes, and provides an auditable record when retained electronically under U.S. e-signature laws (ESIGN/UETA). Many organizations accept electronically signed versions for speed and traceability using secure eSignature platforms such as signNow.

Why use a standardized Remote Work Review Form

A standardized form ensures consistent assessment, reduces disputes over expectations, and creates an auditable trail for compliance with payroll, tax, and data-protection obligations. It centralizes equipment, security, and scheduling details so managers and HR can apply uniform criteria.

Why use a standardized Remote Work Review Form

Who completes and reviews the Remote Work Review Form

The form is completed jointly by the employee and their manager, with HR and IT involvement where appropriate.

  • HR managers and HR generalists — manage policy compliance, record retention, and payroll nexus implications.
  • Direct managers and supervisors — assess role suitability, set performance expectations, and approve or deny requests.
  • Employees requesting remote work — provide personal, role, equipment, and workspace information for assessment.

After approval, HR typically files the signed record in the employee file and schedules any periodic reviews required by policy.

Essential sections to include on the form
