Authorization: Tenant hereby authorizes the Payee and its agent(s) to initiate debit entries to the designated bank account or charge the designated card in the amount and frequency specified in this form. Authorization includes charges for rent, prorated rent, late fees, returned item fees, and other charges as permitted by the lease and applicable law.

Duration and Revocation: This authorization will remain in full force and effect until Payee receives written notice of revocation from Tenant. Revocation will be effective only after Payee has had a reasonable opportunity to act on the notice and will not affect debits previously initiated in reliance on this authorization.

Returned Payments and Fees: Tenant is responsible for any returned item fees, bank charges, and any late fees assessed pursuant to the lease. Payee may assess a charge for a failed electronic transfer as specified in the lease or as otherwise permitted by law.

Representations and Indemnity: Tenant represents that all information provided is true and accurate and that Tenant is authorized to provide instruction to debit the indicated account or charge the indicated card. Tenant agrees to indemnify and hold Payee harmless from losses, damages, claims, or liabilities arising from inaccurate or incomplete information provided by Tenant or from Tenant's failure to comply with the terms of this authorization.

Record of Authorization: Tenant may obtain a copy of this authorization upon request. Tenant consents to electronic or paper records of debits and agrees that such records constitute admissible evidence of the authorization.
