Recitals (WHEREAS)

WHEREAS, Landlord is the lawful owner or authorized manager of the Rental Property identified above and is authorized to grant possession and access to the Tenant; and

WHEREAS, Tenant desires to occupy and/or receive rental services for the Rental Property and acknowledges receipt of the terms and condition set forth in this Rental Acknowledgement Form; and

WHEREAS, the parties intend for this document to memorialize the material terms of the rental arrangement, payment obligations, and acknowledgements related to condition and access.

Scope of Rental Use and Services

The parties agree that the Tenant's permitted use of the Rental Property and Landlord's responsibilities are described below. Tenant acknowledges the following scope and limitations of use:

Payment Terms

Tenant shall pay the amounts set forth below in consideration of possession and services. All sums are due in United States currency unless otherwise stated.

The late fee specified above shall apply if payment is not received within the grace period set by the parties. Interest and collection costs may be charged as allowed by law.

Term and Termination

The rental term and procedures for termination are set forth below. Tenant acknowledges the dates and termination notice requirements.

Either party may terminate this arrangement in accordance with the notice period above and applicable law. Landlord may terminate for nonpayment, material breach, or as otherwise permitted. Tenant shall vacate and return possession in the condition required under this form and any attached inventories.

Tenant acknowledges receipt of the items checked below at move-in and accepts responsibility to return items in substantially the same condition except for normal wear and tear.

Confidentiality

Each party shall hold confidential all non-public information obtained in connection with this rental arrangement, including financial information and security access codes. This obligation survives termination of the rental term for a period of two years.

Governing Law

This Rental Acknowledgement Form shall be governed by and construed in accordance with the laws of the state of , without regard to conflict of laws principles.

Entire Agreement
