Recitals

WHEREAS, Service Provider: (the "Service Provider") is engaged in the business of repair, maintenance, and related services; and

WHEREAS, the Client requests repair services for the item described below and authorizes the Service Provider to perform such services under the terms set forth in this Repair Authorization Form.

Warranty; Limitation of Liability

The Service Provider is not liable for consequential, incidental, or punitive damages, nor for pre-existing conditions or damage due to misuse, neglect, or unauthorized modification.

If the Client fails to collect the item within days after notice of completion, the Service Provider may charge storage fees and, after additional written notice, may dispose of or sell the item to recover costs in accordance with applicable law.
