What a Repair Satisfaction Form Is and when it’s used

A Repair Satisfaction Form is a written acknowledgment completed after repair work is finished that documents the customer’s acceptance of the repair, the work performed, and any remaining issues or follow-up items. It typically includes the repair date, a brief description of the repair or service, parts replaced, labor hours, final cost, and the customer’s signature. Businesses use this form to confirm completion, support billing and warranty claims, and create a contemporaneous record in case of later dispute about performance or condition.

Why documenting customer acceptance matters

A completed Repair Satisfaction Form reduces disputes, supports accurate invoicing, and preserves evidence for warranty or insurance claims.

Common mistakes to avoid when preparing the form

  • Using unclear or vague descriptions of the repair that leave room for later disagreement about the scope of work.
  • Failing to collect a signature or using an unsigned checklist as the sole proof of acceptance.
  • Omitting the final amount or failing to indicate taxes and discounts, which can lead to billing disputes.
  • Not saving a dated, tamper-evident copy of the signed form in a secure records system.
