What a Request Form Is and When It’s Used

A Request Form is a structured document used to initiate a transaction, request services, seek documents, or open an internal approval workflow. It collects consistent data about the requester, the requested items or services, relevant dates, and approval routing. Request Forms are used across operations, procurement, HR, legal, and service desks and can be completed on paper or electronically. Properly completed Request Forms create an auditable record of intent, timelines, and authority, and can be combined with eSignature and authentication to produce enforceable, retained records under U.S. electronic signature law.

Why a Clear Request Form Matters

A well-designed Request Form reduces processing delays, prevents data errors, and creates a consistent audit trail for approvals and later review. It supports legal validity under ESIGN or UETA when electronic consent, attribution, and retention are satisfied.

Why a Clear Request Form Matters

Who Completes and Reviews Request Forms

Typical participants include requesters, approvers, reviewers, and administrative processors who route and act on requests.

  • Requesters: Employees, contractors, or external customers initiating the request for goods, services, or records.
  • Approvers: Managers or budget owners who confirm necessity, availability, and compliance before authorizing.
  • Administrators: Procurement, legal, or records staff who validate data and move the request to fulfillment.

Clear role assignment speeds processing and reduces disputes about authority and accountability.

Primary Roles and Typical Responsibilities

Requester

An individual who completes and submits the Request Form. Responsible for accurate data entry, attaching supporting documents, and providing a valid signature or electronic consent. Errors or missing attachments delay approval and may trigger re-submission.

Approver

A manager or designated approver who reviews the request, verifies budget and policy compliance, and signs or electronically approves. Approval establishes authorization to proceed and becomes part of the retained record for audit and dispute resolution.

Step-by-Step: Filling and Submitting a Request Form

Follow these steps to complete the form, validate data, and route the request for approval.

  • 01
Prepare details: Gather descriptions, codes, and supporting documents before you begin.
  • 02
Complete fields: Enter names, dates and contact information precisely.
  • 03
Attach support: Upload quotes, invoices, or approvals required for review.
  • 04
Sign and submit: Provide signature or electronic consent and send to approver(s).

Typical Routing and Processing Flow

A Request Form usually follows a predictable routing path from initiation through fulfillment.

  • Initiation: Requester creates and signs the form.
  • Approval: Designated approver reviews and authorizes.
  • Procurement: Procurement or service team executes the request.
  • Recordkeeping: Completed form and attachments are stored for retention and audit.
