What the Research Investigator Site Report Is

A Research Investigator Site Report is a structured document prepared by a clinical site to summarize study activities, enrollment, protocol deviations, safety events, regulatory status, and source‑document availability. It consolidates operational metrics and supporting records for sponsors, monitors, and institutional review boards (IRBs). The report typically includes protocol identifiers, visit logs, adverse event summaries, informed consent status, monitoring findings, and a checklist of regulatory documents to support audits and compliance reviews.

Why this report matters for study quality and compliance

A clear site report centralizes evidence required for monitoring, regulatory inspection, and sponsor oversight; it reduces ambiguity about enrollment, deviations, and safety signals while supporting timely corrective actions.

Why this report matters for study quality and compliance

Primary users and audiences for the site report

The Research Investigator Site Report is completed and reviewed by clinical site staff, shared with sponsors and IRBs, and used by monitors and auditors to verify compliance and data integrity.

  • Principal Investigator: responsible for clinical oversight, attestation of data accuracy, and signing key regulatory attestations when required.
  • Study Coordinator / Research Nurse: compiles enrollment numbers, source‑document status, consent forms, and adverse event summaries for the report.
  • Sponsor Monitor / CRA: reviews the report for monitoring visits, verifies corrective action plans, and escalates findings to sponsor safety teams.

Use the report to create an auditable trail of site activities that supports inspections, sponsor decisions, and IRB continuing review.

Core sections to include in a professional site report

A consistent structure improves clarity during monitoring and audits. Include discrete sections so reviewers can locate enrollment figures, safety data, deviations, and regulatory attachments without searching multiple files.

Study Identification

Protocol number, study title, sponsor, site ID, and reporting period; essential for cross‑referencing with sponsor systems and CRFs.

Enrollment Summary

Subject screen/enroll counts, screen failures, withdrawals, active subjects, and any cohort or strata breakdowns required by protocol.

Adverse Events

Summary of serious and non‑serious events, outcomes, relationship assessments, and dates reported to sponsor/IRB.

Protocol Deviations

List of deviations and violations with dates, impact statements, corrective actions, and whether escalation to IRB or sponsor occurred.

Regulatory Documents

Current CVs, medical licenses, IRB approvals, consent form versions, delegation logs, and equipment calibration records.

Data and Source Status

Status of source documents, query backlogs, outstanding CRF pages, and planned timelines to resolve open data items.

Essential fields to capture on every report

Protocol ID: Unique protocol code
Reporting Period: Start and end dates
Site Identifier: Sponsor site number
PI Name: Principal Investigator
Subject Counts: Enrolled/active numbers
Signatures: PI and preparer

Step-by-step: preparing the site report

Follow a repeatable sequence so each report is complete, verifiable, and consistent across reporting periods.

  • 01
Collect Data: Extract enrollment, visit, and AE logs
  • 02
Summarize Findings: Create concise counts and narratives
  • 03
Attach Documents: Include IRB approval and key regulatory files
  • 04
Sign and Submit: Obtain PI signature and route to sponsor/IRB

Configuring an online report workflow

Standardize field settings and signer authentication when you digitize reports to ensure consistent capture and secure delivery.

Field Configuration
Authentication Method Email link or SMS code for signer identity
Conditional Fields Show follow‑up fields only when triggered
Audit Trail Capture IP, timestamp, and signer actions
File Formats Use PDF/A export for archival

Technical and platform considerations for eSubmission

Choose a platform that supports secure authentication, audit trails, and the file formats your sponsor requires.

  • Integrations: Salesforce, NetSuite, Google Workspace compatibility
  • Formats: PDF, DOCX, and Excel supported
  • Security: AES‑256 at rest and TLS in transit

Verify the platform offers HIPAA and 21 CFR Part 11 safeguards if the report includes protected health information or FDA‑regulated records.

Typical submission flow for a finalized report

A predictable submission flow speeds review and reduces back‑and‑forth with monitors and IRBs.

  • Prepare Report: Compile data and attachments
  • Obtain Signatures: PI signs electronically or in ink
  • Route to Sponsor: Send to sponsor contact and CRA
  • Archive Copy: Store signed PDF in site records

Typical timelines to track when reporting

Timelines vary by sponsor and IRB; use the protocol and monitoring plan to confirm exact deadlines for each report.

Initial Site Setup Report:

Submit after site activation or first enrollment

Monthly Enrollment Summary:

Provide counts and screen/failure details monthly

Serious Adverse Event Reporting:

Report per sponsor/IRB timelines and protocol

Monitoring Visit Report:

Send within sponsor‑specified days after visit

Annual IRB Continuing Review:

Submit required materials per IRB calendar

Common preparation mistakes to avoid

  • Incomplete attachments such as missing signed consent forms or expired IRB approvals that trigger sponsor queries and audit findings.
  • Mismatched subject identifiers between the report and source documents, causing reconciliation issues and potential data corrections.
  • Vague descriptions of deviations without dates, impact assessment, or corrective action plans, leading to extended sponsor follow‑up.
  • Relying on screenshots or unsecured file transfers instead of signed, timestamped PDFs with audit trails for regulatory review.

Risks and consequences of inaccurate reports

Regulatory Findings: Inspection observations or warning letters
Sponsor Sanctions: Enrollment holds or site suspension
Data Integrity Issues: Query charges and corrections
Patient Safety Risk: Delayed detection of safety signals
Audit Disallowances: Grant or billable expense rejections
Legal Exposure: Contract disputes or liability claims
