This Resident Information Document (the "Document") is entered into by and between the parties identified below for the purposes of recording resident information, establishing responsibilities for services described in the Scope of Work, and setting forth payment and confidentiality obligations related to occupancy and associated services.

The Manager shall perform the services described below in connection with the premises and the Resident's occupancy. The Scope of Work defines obligations of the Manager and expected Resident responsibilities where applicable.

PAYMENT TERMS

Resident agrees to pay the sums set forth below in full and on time. All payments due under this Document are independent of any other obligations between the parties and are enforceable as provided herein.

All late payments shall accrue interest or late fees as specified above. Manager reserves the right to withhold services and pursue remedies for nonpayment, including administrative remedies and collections, consistent with applicable law.

TERM AND TERMINATION

This Document commences on Start Date: and continues until End Date: unless earlier terminated in accordance with the provisions below.

CONFIDENTIALITY

The parties acknowledge that during the course of performance they may obtain non-public information relating to the other party. Each party agrees to hold such information in strict confidence and not to disclose it to third parties except as required by law or as necessary to perform obligations hereunder. Confidential information does not include information that is or becomes publicly available through no fault of the receiving party.

Resident acknowledges that personal contact and emergency information provided in this Document will be used by Manager for administration, safety, and legal compliance and will be treated in accordance with the confidentiality obligations above.

GOVERNING LAW

This Document shall be governed by and construed in accordance with the laws of the State of without regard to its conflict of laws principles. Venue for any dispute shall lie in the appropriate state or federal court located within that jurisdiction.

ENTIRE AGREEMENT

This Document, together with any attachments or addenda executed by the parties, constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior and contemporaneous understandings and agreements, whether written or oral. No amendment shall be effective unless in writing and signed by both parties.

The Resident represents and warrants that the information provided in this Document is true and accurate to the best of Resident's knowledge. The Resident certifies that any changes to the information provided will be communicated to the Manager in writing within a reasonable time.
