What the Residential Care Facility RCFE Form Is

The Residential Care Facility RCFE Form is the admission and service agreement used by licensed assisted living providers to document a prospective resident's personal information, health and medication needs, services to be provided, fees and payment terms, and required consents. It establishes the contractual relationship between the facility and resident or responsible party, sets out house rules, outlines care levels and emergency procedures, and records contact and legal representative details. Many states use a similar admission agreement for assisted living; the RCFE term is commonly used in California licensing contexts but the form's core elements apply broadly to assisted living admissions nationwide.

Why a Correct RCFE Form Matters

A properly completed Residential Care Facility RCFE Form protects resident rights and clarifies provider obligations by documenting health needs, service levels, fees, and legal authorities. Accurate information supports safe care, billing accuracy, and regulatory compliance under state licensing standards and applicable federal laws such as the Electronic Signatures in Global and National Commerce Act (ESIGN).

Why a Correct RCFE Form Matters

Who Completes and Signs the RCFE Form

The RCFE Form is completed by facility intake staff with input from the prospective resident, responsible family members, or an appointed legal representative.

  • Responsible party or POA — signs financial and decision-making sections when authorized.
  • Facility administrator or intake nurse — verifies items, notes care plan and services.

Keep signed originals per licensing rules and provide a copy to the resident or responsible party; retain digital copies under secure retention policies aligned with HIPAA and state law.
