What the Resignation Application Form Is

A Resignation Application Form is a written notice from an employee informing an employer of the employee's intent to leave their position. It records the employee's name, position, effective date of resignation, and optional reasons or comments. Employers use the form to initiate offboarding steps such as final pay calculations, benefits termination or continuation, equipment return, and personnel record updates. The form may be stored in personnel files and used to confirm legal and payroll obligations at separation.
