What the Revised Task Order Template Is

The Revised Task Order Template is a standardized contract amendment used to change the scope, schedule, price, or administrative terms of an existing task order under a master contract. It memorializes agreed changes in scope, assigns responsibilities for new deliverables, and records revised milestone dates and payment adjustments so parties have a single, auditable source of truth for the modified engagement.

Why Use a Formal Revised Task Order Template

A formal template reduces ambiguity, documents approvals, and creates a traceable record for contract changes. Proper use helps prevent disputes, ensures consistent internal routing, and supports compliance with procurement and audit requirements.

Why Use a Formal Revised Task Order Template

Who Typically Prepares and Signs a Revised Task Order

Several role types commonly prepare, approve, or sign revised task orders depending on organizational structure and procurement rules.

  • Contracting officers and procurement managers — oversee approvals, verify budget availability, and ensure compliance with contract terms and regulations.
  • Program or project managers — define technical changes, update schedules and acceptance criteria, and confirm deliverable revisions.
  • Contractor authorized representatives — accept the revised scope, update resource plans, and confirm payment or milestone adjustments.

Use the organization’s signature authority matrix to confirm who may legally bind each party before routing the template for signatures.

Who Can Sign and Their Typical Roles

Contracting Officer

Authorized procurement official who approves changes to price, period of performance, and key contractual clauses; typically must follow internal delegation and record approvals in the procurement file.

Contractor Representative

Officer or manager authorized by the contractor to accept scope changes, commit resources, and acknowledge revised payment or schedule terms on behalf of the vendor.

Core Elements to Include in a Professional Revised Task Order

A complete revised task order should be clear, concise, and contain the contractual elements needed for enforceability and administration.

Task Order ID

Reference the original task order number and include a unique revision identifier so records and invoices link unambiguously to the correct contract change.

Revised Scope

Describe added or removed tasks with specific deliverables, acceptance criteria, and any dependent assumptions to avoid later disputes over performance expectations.

Schedule Changes

State revised start and completion dates, milestone due dates, and any interim acceptance or inspection points so progress can be tracked against the amended timeline.

Price and Payment

Show the prior price, the change amount (increase or decrease), new total, invoicing schedule, and any retainage, rates, or cost-reimbursement clarifications.

Change Authorization

Identify the approving authority and reference underlying contract clauses or delegation memos that authorize the revision, including any required funding citations.

Acceptance and Signatures

Provide signature blocks for all authorized parties, effective date of the amendment, and any witness or notarization fields required by policy or jurisdiction.
