Employee authorizes Employer and Employer's payroll agent to initiate credits to the identified account for salary payments and to initiate, if necessary, adjustments or debits for any salary overpayment or administrative correction in accordance with company policy.

Employee acknowledges that Employer will rely on the account details supplied herein and that Employer is not liable for transfer failures, delays, or misdirected funds resulting from inaccurate or incomplete information provided by Employee. Employee agrees to indemnify and hold Employer harmless from any claims, losses, or expenses arising from inaccurate account data or Employee's failure to timely notify Employer of changes to account information.

Employee understands that this authorization will remain in effect until revoked in writing by Employee and received by Employer with sufficient time to process the change (no less than fourteen (14) days prior to the next scheduled payment). Employer may terminate direct deposit arrangements upon notice to Employee or as required by banking partners. Employer reserves the right to require a new authorization or additional documentation to effect a change.

Employee authorizes Employer to transmit personal data necessary to effect the transfer to financial institutions and payroll processors. Such transfers will be used solely for processing salary transfers and related payroll administration, consistent with applicable privacy and employment laws.

Any bank fees, returned item charges, or other fees assessed by the financial institution as a result of Employee-provided instructions may be charged to Employee or withheld from future salary payments as permitted by law and company policy.

To prevent fraud, Employee confirms that the account owner name matches Employee's legal name or that Employee is an authorized signer on the account. Employer may require supporting documentation (voided check, bank letter) before accepting this authorization.
