What the Salvation Army Donation Form Is and Why It Exists

The Salvation Army Donation Form documents a donor’s contribution to The Salvation Army or an affiliated program, records donor contact details and gift specifics, and provides the written acknowledgment donors need for tax substantiation when applicable. It serves as the organization’s internal record for gift processing, accounting, and donor stewardship, and can capture restricted gift instructions, fair-market-value statements for non-cash donations, and donor consent for communications. The form is commonly used at donation centers, events, online portals, and when corporate workplace giving is processed.

Why a Clear Donation Form Matters

A well-designed Salvation Army Donation Form ensures accurate tax substantiation, consistent receipting, and reliable donor records while reducing processing errors and delays.

Why a Clear Donation Form Matters

Who Typically Completes or Receives This Form

The form is used by donors, Salvation Army staff, and third-party donation processors; it links the gift to a donor record and specifies any restrictions.

  • Individual donors completing in-person or online donation transactions and claiming tax deductions.
  • Donation intake staff and volunteers recording gift type, condition, and receipt issuance.
  • Corporate giving coordinators and payroll deduction administrators processing workplace contributions.

Accurate completion by the person entering donor details helps ensure valid acknowledgements, proper bookkeeping, and faster reconciliation.

Who Signs or Certifies the Form

Donation Coordinator

Donation coordinators record the donor’s information, confirm gift condition for in-kind items, and sign or initial internal processing fields to verify the intake review and acceptance.

Donor

The donor provides name, address, signature or electronic consent, and gift details; their signature or e-consent attests to intent, which is needed for donor-substantiation and audit trails.

Audit Trail: Timestamps and IP logging
HIPAA BAA: BAA required for PHI handling
Access Controls: Role-based user permissions
Retention: Preserve records per policy
Authentication: Email code or MFA options

  • Authentication: Email or SMS code verification
  • Integrations: Able to connect with CRM or accounting

Platforms that integrate with CRM and finance systems reduce manual entry, improve matching, and speed receipt issuance.

Timing and Deadlines to Keep in Mind

Certain timing practices affect tax substantiation, donor acknowledgements, and year-end reporting; adhere to fund accounting and donor recognition schedules.

Receipt at Time of Gift:

Provide a receipt or acknowledgement when the donation is received.

Substantiation for $250+:

Written acknowledgement required before the donor claims a deduction for large gifts.

Year-End Reporting:

Reconcile gifts with accounting before fiscal year close.

Payroll Deductions:

Process employer payroll gifts per payroll cutoff schedules.

Non-Cash Appraisals:

Obtain required valuation paperwork before issuing a value-based receipt.

Common Pitfalls to Avoid

  • Entering incorrect donor email or name prevents automated receipt delivery and matching.
  • Failing to attach valuation docs for in-kind gifts can disallow claimed donor deductions.
  • Using vague descriptions for gifts (e.g., 'collection of items') complicates accounting and audit review.
  • Not recording restriction instructions may result in funds used inconsistently with donor intent.

Risks and Consequences of Inaccurate Donation Records

Tax Disallowance: Donor deduction may be denied
Audit Exposure: Organization faces increased scrutiny
Penalty Risk: Potential IRS penalties for misreporting
Reputational Harm: Donor trust can erode
Legal Risk: Misapplied restricted funds risk litigation
Refunds: Incorrect receipts can require corrections
