What the Sample Resistor Form Is and When It’s Used

The Sample Resistor Form is a standardized request and specification record used to order, evaluate, or approve resistor samples between suppliers, contract manufacturers, and design teams. It captures technical specifications such as resistance value, tolerance, power rating, package type, and required testing conditions, plus requester contact and intended application. The form creates a clear paper trail for procurement, quality control, and traceability during prototype evaluation or qualifying production lots, helping reduce miscommunication and enabling consistent comparison between multiple supplier offers.

Why a Consistent Sample Resistor Form Matters

A well-designed form reduces errors, shortens evaluation cycles, and documents technical and commercial terms so spec changes, approvals, and results are auditable and reproducible.

Why a Consistent Sample Resistor Form Matters

Core Elements of a Professional Sample Resistor Form

A complete form balances technical detail, commercial terms, test requirements, and clear signatory authority so samples can be produced, evaluated, and approved without repeated clarification cycles.

Supplier Info

Supplier name, contact, and part number are listed to ensure the sample maps to the supplier’s internal catalog and for traceability during follow-up testing and purchase orders.

Resistor Specs

Resistance value, tolerance, power rating, temperature coefficient, and package type should be specified precisely, including units and any preferred manufacturer markings or codes.

Quantity & Packaging

State the sample quantity required for testing, preferred packaging (reel, tape, tube), and any anti-static handling or moisture-sensitivity packaging needs.

Test Requirements

List required performance tests, test conditions, acceptance criteria, number of samples per test, and whether test reports or certificates of compliance are required.

Commercial Terms

Specify who pays shipping, unit sample cost, lead times, any return requirements, and whether the sample is free, credited, or billable upon approval.

Approval & Signoff

Include signature blocks for the requester, technical approver, and procurement with printed name, title, date, and an explicit statement of approval or rejection criteria.

Step-by-Step: How to Fill Out the Form

Complete the form in the following order to minimize rework and ensure supplier acceptance.

  • 01
Enter requester details: Provide full contact and procurement information.
  • 02
Specify resistor specs: Add resistance, tolerance, power rating, package.
  • 03
Describe tests: List test types, conditions, and acceptance criteria.
  • 04
Sign and date: Approver signs and dates to authorize sample production.

How to Configure an Online Form Workflow

Example digital workflow settings to streamline sample requests, approvals, and test-report collection.

Field Configuration
Auto-detect fields Enable magic detection for names and dates
Conditional fields Show test fields only when specific tests selected
Signer authentication Use email or SMS code verification
Audit trail Capture IP, timestamp, and action log

Digital Signing and File Formats to Use

Choose a platform that supports PDF and DOCX uploads, audit trails, and basic signer authentication for traceability.

  • Supported formats: PDF, DOCX, XLSX
  • Authentication: Email link or SMS code
  • Audit trail: Timestamp and IP capture

Where Completed Forms Should Be Sent

Follow this routing pattern to ensure timely sample production and testing.

  • Submit to supplier: Send completed form to supplier account manager
  • Notify procurement: CC procurement for P.O. reconciliation
  • Route to lab: Send copies to test lab with instructions
  • Archive copy: Store signed form in company records system

Teams and Roles That Commonly Complete This Form

Coordinate a single owner per request—typically the requesting engineer or procurement buyer—to manage communications and approvals.

  • Design engineers request samples for prototype verification and tolerance checks during development stages.
  • Procurement and sourcing teams use the form to obtain supplier quotes and create purchase orders after approval.
  • Quality and test labs receive the form with explicit test instructions to validate performance and acceptance criteria.

Typical Signatories and Their Responsibilities

Design Engineer

The Design Engineer specifies technical requirements, approves test criteria, and evaluates sample performance. They are responsible for technical signoff and raising change requests if samples do not meet criteria.

Procurement Manager

The Procurement Manager confirms supplier commercial terms, issues purchase orders if samples are billable, and coordinates shipping and invoicing reconciliation with accounts payable.

Essential Fields and Data You Must Provide

Part Number: Supplier part number
Resistance: Value and units
Tolerance: Percent or ppm
Power: Watts rating
Quantity: Number of pieces
Contact: Name, email, phone

Common Mistakes That Delay Sample Delivery

  • Leaving units unspecified (ohms vs kilo-ohms) causes supplier confusion and incorrect parts being shipped.
  • Omitting tolerance or power rating forces suppliers to guess a default, which can invalidate test results.
  • Requesting tests without acceptance criteria leads to repeated cycles of clarification and retesting.
  • Not specifying billing or shipping responsibility causes procurement and supplier invoice disputes.

Risks When the Form Is Incomplete or Incorrect

Order Delay: Supplier needs clarification
Wrong Parts: Delivered components mismatch specs
Test Failures: Results unusable without accepted criteria
Cost Overruns: Unexpected sample charges
Procurement Hold: PO reconciliation stalls production
Traceability Loss: Unable to link sample to lot

Typical Timelines and Processing Expectations

Use these target timeframes to set expectations with suppliers and internal teams for sample delivery and evaluation.
