What the Security Clearance Reinstatement Form Is

The Security Clearance Reinstatement Form is the official request used to restore or reestablish an active security clearance that has lapsed, been suspended, or administratively closed. It compiles applicant identification, prior clearance history, reasons for lapse, and any intervening changes in personal or professional circumstances. Agencies use the form to reopen background investigations, evaluate eligibility under adjudicative guidelines, and determine whether clearance levels and access rights should be restored. The form is submitted to the sponsoring organization or the relevant security office and becomes part of the personnel security record for adjudication and audit purposes.

Why a Properly Completed Reinstatement Form Matters

Completing the Security Clearance Reinstatement Form accurately helps agencies assess eligibility quickly, avoids unnecessary delays, and preserves employment or contract access that depends on cleared status.

Why a Properly Completed Reinstatement Form Matters

Who Typically Prepares and Reviews This Form

Final review normally occurs with the sponsoring security office; ensure all supporting records are attached to prevent processing delays.

  • Sponsoring Organizations: Human resources or facility security officers who request adjudicative review on behalf of employees or contractors.
  • Applicants: Individuals who previously held a clearance and must report changes in circumstances since the last adjudication.
  • Security Offices: Personnel security specialists and adjudicators who evaluate background information and determine reinstatement eligibility.

Stepwise Process for Completing the Form

Follow these sequential actions to prepare a complete reinstatement submission and reduce the chance of administrative return.

  • 01
Gather Documents: Collect prior clearance records and ID documents.

Complete Form: Enter all fields, follow format instructions.

Key Sections That Determine Adjudication Speed

Understanding the form's major sections helps applicants provide focused, verifiable information that supports a timely reinstatement decision.

Applicant Identification

Collects name, date of birth, social security number, and other identifiers used to match records and perform background checks; accuracy here prevents duplicate files.

Clearance History

Records prior clearance level, granting authority, dates, and reason for termination; this history directs the adjudicative path and determines whether a full reinvestigation is required.

Employment and Affiliations

Captures current and recent employers, foreign contacts, and organizational memberships that adjudicators consider under suitability and foreign influence guidelines.

Legal and Financial Issues

Discloses arrests, convictions, bankruptcies, and delinquent financial obligations which can materially affect eligibility under the adjudicative standards.

Supporting Evidence

Space for attachments: letters from supervisors, records of rehabilitation, or documentation resolving issues cited in the applicant record to expedite adjudication.

Certification and Signature

Applicant attestation of truthfulness; includes signature block and date — false statements may have administrative and legal consequences.

How to Configure an Online Reinstatement Workflow

Digital workflows reduce manual handoffs and preserve an audit trail required for adjudication and compliance.

Field Configuration
Applicant ID Field Required, read-only verification
Attach Evidence Field Allow PDF/JPG uploads
Signature Field eSignature with audit trail
Routing Rules Route to FSO then adjudicator

  • Applicant Submission: Applicant signs and uploads form.
  • Sponsor Review: Sponsoring official verifies completeness.
  • Security Office: PSO forwards to adjudication team.
  • Adjudicative Decision: Agency issues reinstatement or denial.

Digital Signing and eSubmission Requirements

For HIPAA or sensitive records, choose a solution offering audit trails, encryption, a BAA if required, and retention controls aligned to agency policies.

  • File Formats: Accept PDF and DOCX formats
  • Authentication: Email, SMS, or advanced methods
  • Integrations: Supports Salesforce, NetSuite, Box

Timing Considerations and Typical Deadlines

Processing times and internal deadlines vary by agency and case complexity; use these benchmarks when planning submissions and follow-up.

Varies widely; expect weeks to several months

Sponsor Internal Deadlines:

Employers often set internal submission windows

Document Currency:

Provide the most recent records available

Follow-up Intervals:

Check status with FSO every 30–60 days

Timelines set by agency policies

Common Preparation Mistakes to Avoid

  • Incomplete prior-clearance details that prevent record matching and delay adjudication.
  • Missing or illegible supporting documents, such as supervisor letters or court records, requiring supplemental requests.
  • Mismatched identity data (name, DOB, SSN) between form and government IDs causing identity verification failures.
  • Using informal or vague explanations for issues that require precise dates, locations, and corroborating evidence.

Consequences of Errors or False Statements

Reinstatement Denial: Administrative denial or longer ineligibility
Access Suspension: Loss of classified access until resolved
Employment Impact: Contract termination or reassignment
Criminal Exposure: False statements can trigger prosecution
Investigation Delay: Additional interviews and checks
Financial Costs: Attorney fees and administrative costs

Practical Tips to Speed Reinstatement Review

Follow these practical steps to reduce administrative friction and increase the chance of a timely adjudication.

Verify Identity Details
Cross-check name, DOB, and SSN against government ID before submission; consistent identifiers prevent record-matching problems and follow-on delays.
Attach Corroborating Records
Include employer letters, court dispositions, and financial reconciliations where relevant; corroboration often accelerates adjudicative decisions.
Use Secure eSigning
Choose platforms that provide an auditable trail, encryption at rest and in transit, and a BAA if PHI is included.
Keep Clear Explanations
Provide concise, dated narratives for gaps or adverse events; clear explanations with evidence reduce the need for supplemental requests.

Authorized Signatories and Their Roles

Facility Security Officer

The FSO or sponsoring official typically verifies completeness, signs sponsor attestations, and forwards the reinstatement package to the adjudicating authority; they act as the primary organizational contact.

Applicant

The individual seeking reinstatement must attest to the accuracy of disclosures, sign where required, and supply supporting documentation; failure to sign or to provide required evidence can halt processing.
