What a Service Call Request Is and When It’s Used

A Service Call Request is a written record that a customer or internal stakeholder submits to request on-site or remote service, repair, inspection, or maintenance. The form documents contact and location details, a concise description of the issue, preferred scheduling windows, access instructions, and any authorization to proceed. It creates a traceable request that dispatch, billing, and warranty teams can act upon and use to confirm completion, costs, and follow-up actions while preserving an audit trail for compliance and customer service records.

Why a Clear Service Call Request Matters

A complete Service Call Request reduces response times, prevents miscommunication, and supports accurate billing. It provides a documented authorization to enter premises or perform work and creates a consistent handoff between customer service, field technicians, and accounts.

Why a Clear Service Call Request Matters

The Service Call Request is completed by and routed among several roles depending on the organization and the service type.

  • Customer or tenant requesting service, providing contact details and access instructions for the technician.
  • Dispatch or service coordinator who schedules the appointment and assigns the technician.
