Recitals

WHEREAS, Client and Service Provider entered into the Original Agreement identified above under which Service Provider agreed to perform certain services for Client; and

WHEREAS, the parties desire to amend the terms of the Original Agreement as set forth in this Service Modification Form to reflect changes to the scope, schedule, and compensation for services; and

WHEREAS, except as expressly modified herein, the Original Agreement remains in full force and effect and the parties intend that this modification supersede only those terms expressly amended below.
