SERVICE REPAIR AUTHORIZATION FORM

Parties

Recitals

WHEREAS, Service Provider is engaged in the business of diagnosing, servicing, and repairing equipment and agrees to perform repair services under the terms set forth herein; and

WHEREAS, Client owns or is responsible for the equipment described below and has requested Service Provider to perform repair and related services, and agrees to authorize such repairs subject to the limitations and conditions of this Authorization; and

WHEREAS, the parties desire to set forth the scope, payment terms, and other material terms governing the performance of repairs and any related services.

Equipment & Problem Description

Scope of Work

Service Provider shall perform the diagnostic, repair and replacement services described below. The Scope of Work shall include labor, installation of identified replacement parts, testing, and reasonable cleanup upon completion. Any work outside the Scope of Work requires separate written authorization.

Payment Terms

Client agrees to pay Service Provider for labor, parts, and applicable taxes in accordance with the terms below. All estimates are approximate; Client will be notified of material deviations and must authorize additional charges when required.

Late payments shall accrue interest at the rate specified above, compounded monthly, and Client shall be responsible for reasonable collection costs and attorney's fees incurred to collect delinquent amounts.

Term and Termination

This Authorization shall commence on the Start Date and shall continue until completion of the Scope of Work or earlier termination in accordance with this Section.

Either party may terminate this Authorization for material breach by the other if such breach remains uncured for the notice period specified above. Termination shall not relieve Client of obligations to pay for services performed or parts ordered prior to termination.

Authorization & Consent

By checking the boxes below and signing, Client authorizes Service Provider to perform the repairs and to obtain necessary parts. Client also authorizes Service Provider to dispose of replaced parts unless Client requests return of parts and assumes any associated cost.

Warranties; Liability

Service Provider warrants that its workmanship will be performed in a skillful and workmanlike manner for a period of days from completion. Parts warranties are limited to the manufacturer's warranty. Except as expressly provided herein, Service Provider disclaims all other warranties, express or implied.

Client acknowledges that Service Provider's total liability for any claim arising out of or related to the services shall not exceed the amounts paid by Client to Service Provider for the specific services giving rise to the claim. Under no circumstance shall Service Provider be liable for incidental, consequential, or punitive damages.

Confidentiality

Each party agrees to keep confidential and not disclose to third parties any non-public business or technical information obtained in connection with the performance of this Authorization, except as necessary to perform the services or as required by law. Confidential information does not include information that is or becomes publicly available through no fault of the receiving party.

Governing Law

This Authorization shall be governed by and construed in accordance with the laws of the jurisdiction specified below, without regard to conflict of laws principles.

Entire Agreement

This Authorization, including any written estimates and attachments signed by the parties, constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings. Any modification must be in writing and signed by both parties.

Acknowledgment

Client acknowledges receipt of the estimate and authorizes the Service Provider to perform the repairs in accordance with the terms herein. Client certifies that the information provided is true and that they have authority to authorize repairs on the described equipment.
