What a Shipping Information Form Is and when it's used

Shipping Information Form is a standardized document used to collect shipment details required to move goods between parties, including sender and recipient addresses, carrier choices, delivery instructions, package descriptions, quantities, and tracking information. It organizes contact and billing information, customs declarations for international shipments, and signature fields for authorization. Organizations use it to ensure accurate fulfillment, confirm carrier responsibilities, and create a record for billing and compliance. When completed electronically, the form can include audit trails and timestamps to support dispute resolution and regulatory recordkeeping.

Why a clear Shipping Information Form matters

A Shipping Information Form reduces delivery errors, standardizes carrier instructions, and documents consent and charge authority. Electronically completed forms speed processing, improve traceability with audit trails, and help meet regulatory or customs documentation requirements.

Why a clear Shipping Information Form matters

Essential sections to include on a professional form

A professional Shipping Information Form includes clear sections for parties, shipment details, customs data, carrier instructions, billing, and authorization to ensure reliable execution and compliance.

Parties

List full legal names, departmental contacts, phone numbers, and email addresses for sender, recipient, and any third-party billing contacts. Accurate party identification supports carrier claims and financial reconciliation.

Addresses

Include street address, city, state, ZIP, and country. For international shipments provide EORI or tax IDs when applicable. Avoid P.O. boxes for deliveries that require customs inspection or curbside delivery.

Package Details

Describe contents with HS codes when applicable, item quantities, dimensions, weight, and declared value. Clear descriptions reduce customs delays and support accurate carrier handling and claims processing.

Carrier Instructions

Specify carrier name, service level, requested pickup date, delivery window, and required handling (fragile, temperature control). Include insurance and declared value instructions to define carrier responsibilities.

Customs & Compliance

Provide harmonized system codes, country of origin, export licenses, and any restricted party screening results. Attach commercial invoices for international shipments to meet customs requirements.

Authorization

Include signature block with printed name, role, date, and any required witness or notary lines. Authorization language should state permission to ship and charge agreed accounts.
