What the Site Design Comment Template Is and When It’s Used

The Site Design Comment Template is a structured feedback form used to capture review notes, change requests, and approval status for a website or physical site design deliverable. It standardizes reviewer input — design intent, accessibility concerns, content corrections, asset requests, and implementation priorities — so stakeholders, designers, and developers can act on a single, auditable record. The template is intended for iterative review cycles during design sprints, client approvals, internal QA, and handoffs to engineering or construction teams where clear, versioned commentary prevents miscommunication and rework.

Why a Template Improves Design Review Outcomes

Using a consistent Site Design Comment Template reduces ambiguity, shortens review cycles, and makes decisions traceable for future audits or disputes.

Why a Template Improves Design Review Outcomes

Who Typically Completes This Template

The template is useful across cross-functional teams and external reviewers who must provide clear, actionable feedback.

  • Designers and UX leads who record visual and interaction issues for the development team.
  • Product managers and stakeholders who consolidate business requirements and sign-off items.
  • Quality assurance and accessibility testers who log defects, contrast problems, and compliance notes.

Use the template to centralize comments from all reviewers, ensuring each item has an assignee, priority, and expected completion date.

Core Sections to Include in a Professional Comment Template

A complete Site Design Comment Template organizes feedback so items are actionable, prioritized, and assigned. It should make status and history obvious for every entry.

Issue ID

Unique identifier for each comment to enable traceability across versions and tickets.

Location

Page, screen, URL, or drawing reference plus viewport or sheet number where the issue appears.

Description

Concise statement of the problem, expected behavior, and suggested remedy for implementers.

Severity / Priority

Severity scale (Critical, High, Medium, Low) and business priority for scheduling work.

Assignee

Name and role of the person responsible for resolution, with contact info when needed.

Status & Dates

Open, In Progress, Resolved, Closed with created and resolved dates for auditability.

Step-by-Step: Using the Template During a Review

Follow this sequence to run an efficient design review and convert comments into tracked work.

  • 01
Prepare: Share the latest design file and the filled template with reviewers before the meeting.
  • 02
Capture: Record each observation as a separate entry with location and screenshot.
  • 03
Assign: Assign ownership and set a priority and due date for each comment.
  • 04
Track: Update status and resolution notes as changes are implemented and verified.

Configuring an Online Comment Workflow

Map template fields to your document or project management system to automate routing and notification.

Field Configuration
Issue ID Auto-generate sequential IDs
Assignee Map to user directory or email
Attachments Allow PDFs, PNGs, and JPGs
Status Trigger notifications on status change

Where to Send Completed Templates and How They Move Through the Process

Define a clear routing path so completed templates enter the correct ticketing or archival system.

  • Submit to PM: Send the completed template to the project manager for triage and ticket creation.
  • Create Tickets: PM or integration converts each comment into a tracked task in the issue tracker.
  • Implement: Developers or contractors implement fixes and upload updated assets.
  • Verify: Reviewer confirms resolution and updates the template status to Closed.

Digital Tools and Integration Considerations

Choose platforms that accept PDF/Word inputs, support attachments, and integrate with your ticketing system.

  • File formats: PDF, DOCX, and exported image formats are standard
  • Integrations: Common integrations: Salesforce, Microsoft 365, Google Workspace, NetSuite
  • Authentication: Use SSO for consistent user identity mapping

Ensure your chosen tool preserves audit history and enables export to common archival formats for compliance and future reference.
