WHEREAS, the Company named above requires collection of the Individual's Social Security Number for lawful business purposes including tax reporting, payroll processing, and identity verification; and

WHEREAS, the Individual agrees to provide the Social Security Number to the Company for the stated purposes and to the terms and conditions set forth herein; and

WHEREAS, the parties desire to memorialize the collection, permitted uses, retention and protection of the Social Security Number and to set forth related business terms.

Scope of Work

The Company shall use the Individual's Social Security Number only for the purposes described in this Agreement and for activities reasonably related to the business relationship (e.g., payroll, tax filing, benefits administration, background checks, and government reporting). Payment Terms

In exchange for services provided under this Agreement, the Company will compensate the Individual as set forth below.

Term and Termination

This Agreement commences on Start Date: and shall continue until End Date: unless earlier terminated in accordance with this section.

Either party may terminate this Agreement upon written notice to the other party delivered no fewer than days prior to the effective termination date. Termination shall not relieve either party of obligations incurred prior to the effective date of termination.

Confidentiality and Security

The Company shall implement and maintain reasonable administrative, physical, and technical safeguards designed to protect the confidentiality of the Social Security Number and to prevent unauthorized access, disclosure, alteration, or destruction. Access to the Social Security Number shall be limited to personnel with a legitimate business need to accomplish the permitted purposes. In the event of a security breach involving the Social Security Number, the Company shall notify the Individual in accordance with applicable law and shall take reasonable steps to mitigate harm.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to principles of conflicts of law.

Entire Agreement; Severability

This Agreement constitutes the entire understanding between the parties with respect to the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings, whether written or oral. If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Acknowledgment and Certification

By signing below, the Individual certifies under penalty of perjury that the Social Security Number provided is true and correct to the best of the Individual's knowledge, and that the Individual has authority to provide the information herein. The Individual acknowledges that misuse of a Social Security Number may result in civil or criminal liability under applicable law.
