What a Software Risk Assessment Covers

A Software Risk Assessment is a structured evaluation that identifies threats, vulnerabilities, and potential impacts to a software system. It documents assets, threat sources, existing controls, likelihood and impact ratings, and residual risk to support prioritized mitigation and governance decisions across development, operations, and compliance teams.

Why Conduct a Software Risk Assessment

A formal assessment helps organizations prioritize security investments, demonstrate due diligence, and reduce the probability of incidents that could cause operational, regulatory, or financial harm.

Why Conduct a Software Risk Assessment

Teams That Typically Complete This Assessment

Software Risk Assessments are completed by cross-functional teams that balance security, product, and legal concerns.

  • Security and engineering teams who map technical vulnerabilities and mitigation tasks across the codebase and infrastructure.
  • Product owners and project managers who align remediation priorities with release schedules and business risk tolerances.
  • Legal, privacy, and compliance staff who verify controls for HIPAA, FERPA, or contractual obligations where applicable.

Results feed stakeholders for resource planning, board reporting, regulatory reviews, and third-party vendor oversight.

Key Roles Responsible for Assessment

CISO

The Chief Information Security Officer sponsors the assessment program, validates risk criteria, and approves remediation budgets. The CISO uses aggregated findings to report enterprise risk posture to executive leadership and boards.

Product Lead

Product or engineering leads assign owners to specific findings, confirm functional impact of mitigations, and schedule work in development sprints while balancing business requirements and security priorities.

Security and Compliance Controls to Document

Encryption: TLS 1.2/1.3 in transit, AES-256 at rest
Access Controls: Role-based access and least privilege
Audit Logging: Tamper-evident logs with timestamps
HIPAA/BAA: BAA required for PHI handling
Certifications: SOC 2 Type II and ISO 27001
Authentication: 2FA and strong password policies

Consequences of Missing or Incomplete Assessment

HIPAA Violation: Civil fines and corrective action
Data Breach Fines: State law penalties and notifications
Contract Breach: Damages and indemnity claims
Regulatory Penalty: Enforcement actions and audits
Litigation Risk: Class actions or customer suits
Operational Loss: Service outages and remediation costs

Common Pitfalls to Avoid

  • Incomplete asset inventory: missing libraries or third-party services can hide critical vulnerabilities and invalidate risk ratings.
  • Overly broad ratings: failing to define likelihood and impact thresholds causes inconsistent prioritization across teams.
  • No owner assigned: leaving findings unowned delays remediation and increases exposure to incidents or compliance failures.
  • Weak evidence: undocumented controls or missing logs make it impossible to demonstrate remediation during audits or incident reviews.

Core Sections of a Professional Assessment

A comprehensive Software Risk Assessment includes structured sections so results are repeatable, auditable, and actionable for technical and nontechnical stakeholders.

Scope

Define systems, components, and third-party integrations included in the review, plus excluded items and reasons for exclusion.

Asset Inventory

List tangible and intangible assets such as codebases, APIs, data stores, secrets, and deployed environments.

Threat Modeling

Identify threat sources, attack vectors, and misuse scenarios that could exploit system weaknesses.

Control Assessment

Document existing technical, administrative, and physical controls and evaluate their effectiveness.

Risk Scoring

Apply a consistent likelihood and impact matrix to produce prioritized risk ratings for each finding.

Remediation Plan

Assign owners, target completion dates, and acceptance criteria for each remediation task.

Step-by-Step: Completing a Software Risk Assessment

Follow a repeatable sequence to collect evidence, evaluate controls, assign owners, and document residual risk for each finding.

  • 01
Inventory Assets: Enumerate services, data stores, and dependencies to define assessment boundaries.
  • 02
Identify Threats: Map threat agents and attack vectors relevant to the assessed system.
  • 03
Evaluate Controls: Review technical and administrative controls, testing where appropriate.
  • 04
Document Findings: Record risk ratings, owners, remediation steps, and verification criteria.

Typical Workflow for Submission and Review

A standard eSubmission workflow moves the assessment from draft to approved, with audit logs and notifications at each step.

  • Upload Draft: Author uploads assessment document or fills the online form.
  • Assign Reviewers: Designate security, product, and legal reviewers with clear deadlines.
  • Review & Comment: Reviewers annotate findings and request clarifications if needed.
  • Approve and Archive: Approver signs off and the final report is archived with an audit trail.

Configuring the Online Assessment Workflow

Set workflow fields and automation to reduce manual handoffs and ensure traceability across reviewers and signers.

Field Configuration
Conditional Sections Show remediation details only for High risks to simplify reviewer view.
Reviewer Routing Sequential or parallel reviewer order with SLA-based reminders.
Authentication Require MFA or SMS code for sensitive or regulatory reviews.
Notifications Email and in-app notifications for assigned owners and reviewers.

Technical Requirements for eSubmission and Signing

Confirm platform integrations, file formats, and authentication methods before publishing the assessment form.

  • File Formats: PDF, DOCX accepted for templates and signed archives
  • Integrations: Connectors to Salesforce, NetSuite, Microsoft 365 available
  • Authentication: Support for SSO, 2FA, and optional KBA for high-risk signers

Use platform capabilities to attach evidence, capture audit trails, and retain signed records in secure storage.

Practical Tips for Accurate and Efficient Assessments

Adopt consistent templates, assign clear owners, and use automation to reduce errors and speed verification.

Use a Standard Template
A reusable template enforces fields, reduces ambiguity, and ensures completeness across assessments and reporting cycles.
Assign Clear Owners
Every finding should have a named owner, a target remediation date, and measurable acceptance criteria for verification.
Attach Evidence
Link logs, scan results, or configuration artifacts directly to findings to simplify validation and audits.
Automate Notifications
Use automated reminders and escalation rules to keep remediation on schedule and visible to stakeholders.

Typical Timelines and Review Deadlines

Set clear deadlines for each phase from initial assessment to remediation verification to maintain governance and compliance schedules.

Initial Assessment:

Complete within 2–6 weeks depending on scope and system complexity

Remediation Plan:

Published within two weeks of finalizing findings

Management Review:

Executive summary and risk acceptance within one month

Re-assessment:

Target verification after remediation, typically 30–90 days

Archival:

Archive signed reports and evidence after acceptance and auditing

Illustrative Examples from Practical Deployments

Real-world examples show how assessments drive remediations, cross-team collaboration, and audit readiness.

Optica Ventures

Brian Fitzgibbons found the interface simple and easy to use for internal teams

  • This reduced turnaround on security reviews
  • The result was faster signoff from customers and clearer evidence trails for audits and vendor reviews.

Xerox NetSuite

Kodi-Marie Evans needed flexible integrations with ERP systems

  • The assessment linked findings to NetSuite tickets
  • That integration helped prioritize fixes and ensured the legal and operations teams could validate mitigation during contract renewals.
