What the Spa Services Addendum Covers

The Spa Services Addendum is a concise attachment to a client services agreement that specifies treatments, session lengths, pricing, package terms, and any service-specific policies. It records client health disclosures, contraindications, pre- and post-care instructions, cancellation and rescheduling rules, gratuity or payment handling, and limits of liability or warranty for services rendered. Providers use the addendum to document informed consent for treatments that have medical or safety implications and to create a clear record that becomes contractually binding when signed by the client and an authorized representative of the spa.

Why add a Spa Services Addendum to your contracts

A Spa Services Addendum reduces disputes by detailing what services will be provided, how pricing and cancellations work, and what medical disclosures clients must make. It clarifies responsibilities, supports compliance with consumer disclosure rules, and creates an auditable record for both parties.

Why add a Spa Services Addendum to your contracts

Who typically completes a Spa Services Addendum

Spas, day spas, medical spa clinics, and independent therapists use this addendum to record treatments and client-specific instructions before services begin.

  • Spa owners and managers who must document service terms and policies for customers.
  • Licensed practitioners (estheticians, massage therapists, medical staff) documenting client consent and medical disclosures.
  • Front-desk administrators who intake client details, schedule sessions, and record cancellations or reschedules.

Use the addendum when services deviate from a standard menu, involve medical risk, or require client acknowledgements prior to treatment.

Primary signers and their roles

Client

The individual receiving services. Signs to confirm health disclosures, consent to specified treatments, and agreement to payment and cancellation terms. Must provide accurate identifying information.

Authorized Rep

A spa owner, manager, or licensed practitioner who verifies service details, confirms practitioner qualifications, and signs to accept operational terms and any service limitations on behalf of the provider.

Core elements to include in a professional Spa Services Addendum

A complete addendum combines treatment specifics, client health disclosures, pricing and cancellation rules, liability language, practitioner credentials, and signature blocks so both parties have a clear, enforceable record of expectations.

Treatment Details

List the exact procedure name, duration, product types, and any sequence of steps so the scope of services is unambiguous and measurable for both parties.

Client Health Disclosures

Request allergies, skin conditions, medications, recent procedures, pregnancy status, and other contraindications to document informed consent and reduce treatment risk.

Pricing & Fees

Specify per-treatment price, package discounts, taxes, gratuity policy, accepted payment methods, and any deposits or nonrefundable fees required to secure appointments.

Cancellation Policy

State time windows for free cancellation, late-cancellation fees, rescheduling rules, and how no-shows are handled to set clear financial expectations.

Liability & Limitations

Include disclaimers about results, a brief limitation of liability, and any client obligations such as following pre/post-care instructions to support realistic expectations.

Signatures

Provide dated signature lines for client and provider, printed names, license numbers where relevant, and an acceptance statement tying the addendum to the primary service agreement.

Step-by-step: completing the Spa Services Addendum

Follow a straightforward sequence to gather client information, confirm service specifics, and capture signed consent before treatment begins.

  • 01
Collect ID: Verify client identity against government ID before accepting medical disclosures.
  • 02
Record Health Data: Enter allergies, medications, and contraindications into the form checklist.
  • 03
Confirm Services: Select exact treatments, quantities, and scheduled dates.
  • 04
Sign & Store: Obtain client signature and save the executed addendum to the client file.

Configuring an online addendum workflow

Set up fields, authentication, and routing so the addendum is completed accurately, securely, and stored with the client record.

Field | Configuration Field name | Field behavior and validation
Client Name Required text field with exact-match validation
Date Fields MM/DD/YYYY with calendar picker
Health Checklist Checkbox group with conditional notes field
Signature E-signature field with signer email and timestamp

Where to send or file the completed addendum

Decide in advance whether the executed addendum is stored in a client management system, emailed to the client, or retained in a secure records folder.

  • Client Record: Attach executed addendum to the electronic client file for future reference.
  • Email Copy: Send a signed PDF to the client’s email address for their records.
  • Point-of-Service: Keep a printed copy in the appointment file when required for in-person verification.
  • Billing System: Link the addendum to invoices or package records for accurate billing.

Digital signing and platform considerations

Choose a platform that supports secure eSignature, field validation, and auditable trails for consumer-facing health-related forms.

  • Authentication: Email verification or SMS code
  • Security: TLS in transit and AES-256 at rest
  • Recordkeeping: Immutable audit trail and signed PDF

Ensure the platform can produce a signed PDF and maintain access to the record for the required retention period and regulatory needs.

Key risks and consequences of an incorrect addendum

Consumer Disputes: Ambiguous terms can lead to refund demands or chargebacks
Licensing Issues: Missing practitioner details can trigger licensing complaints
Medical Liability: Incomplete health disclosures increase malpractice exposure
Tax Consequences: Inadequate records may complicate reporting of gift certificates or prepaid packages
Record Retention: Failure to retain records risks noncompliance with audits
Signature Validity: Improper e-sign setup may weaken enforceability

Common mistakes when preparing a Spa Services Addendum

  • Using vague treatment descriptions that do not match industry-standard procedure names, which leads to scope disputes and client confusion.
  • Failing to collect or record accurate medical disclosures, increasing safety risks and potential liability for adverse outcomes.
  • Not specifying cancellation or deposit rules clearly, resulting in inconsistent enforcement and customer disputes over charges.
  • Keeping signed forms only in paper files without backup, which risks loss, audit gaps, or inability to produce records when needed.

Practical tips for accurate and efficient completion

Follow these practices to reduce risk, accelerate intake, and keep client records organized and compliant.

Use standard service codes

Implement templated language
Use a vetted template for recurring services and route any custom clauses to legal review before use.
Store signed PDFs securely
Keep executed addenda in encrypted storage with access controls and an auditable access log.
