What the Specimen Submission Form Is and When it’s Used

A Specimen Submission Form documents the chain-of-custody, clinical details, and administrative instructions for biological or environmental samples sent to a testing laboratory. It typically records patient or submitter identification, sample type and container, collection date/time, tests requested, billing or insurance details, and any special handling instructions. Laboratories, clinics, research teams, and couriers use the form to ensure correct processing, prevent misidentification, and provide traceability. Accurate forms reduce repeat collections and support regulatory or accreditation reviews.

Why a Complete Specimen Submission Form Matters

A correctly completed form preserves sample integrity, speeds laboratory processing, and reduces diagnostic delay. It supports regulatory compliance and accurate billing while creating an auditable record for chain-of-custody and patient safety.

Why a Complete Specimen Submission Form Matters

Who Typically Completes and Receives This Form

The Specimen Submission Form sits at the intersection of clinical care, laboratory operations, and logistics; several roles routinely prepare or receive it.

  • Clinicians and nursing staff preparing samples for diagnostic testing and documenting clinical context.
  • Laboratory intake teams verifying identifiers, accessioning samples, and initiating testing workflows.
  • Medical records or billing staff associating orders with insurance or account information.

Typical Signers and Their Responsibilities

Lab Manager

Oversees sample acceptance criteria, audits chain-of-custody records, and enforces labeling and documentation standards. Responsible for rejecting nonconforming specimens and communicating required recollection steps to clinical teams.

Ordering Clinician

Orders tests, confirms clinical information and patient identifiers, signs to authorize testing, and provides pertinent clinical history. Responsible for ensuring consent is obtained when required by law or institutional policy.

Step-by-Step: Completing and Submitting the Form

Follow these steps in order to prepare the specimen and deliver the form with consistent, auditable documentation.

  • 01
Step 1: Verify Identity: Confirm patient identity and match identifiers across specimen and form.
  • 02
Step 2: Label Specimen: Affix a unique barcode or handwritten ID matching the form.
  • 03
Step 3: Complete Fields: Enter all required fields, including tests and clinical history.
  • 04
Step 4: Package and Ship: Use appropriate biohazard packaging and select correct transport conditions.

How the Submission Workflow Typically Flows

A standard submission moves from collection through transport to laboratory intake; each stage captures timestamps and personnel involved.

  • Collection: Sample is collected and initial identifiers recorded.
  • Labeling: Specimen receives a unique ID matched to the form.
  • Transport: Courier or internal transport maintains required temperature.
  • Laboratory Intake: Intake team verifies form, logs receipt, and assigns accession number.

Core Elements of a Professional Specimen Submission Form

A complete form groups administrative, clinical, and processing instructions so lab staff and clinicians can act without delay or additional outreach.

Patient Identification

Full patient or submitter legal name, date of birth, medical record number, and contact details to ensure correct record linkage and reduce the risk of misidentification during processing.

Specimen Details

Precise specimen type, container, volume, and unique sample identifier or barcode so laboratory staff know how to handle, store, and process the sample according to testing requirements.

Collection Information

Collection date, time, collector name or initials, and collection site to establish sample age and suitability for time-sensitive assays or stability-dependent testing.

Test Orders

Explicit tests requested by name or code, priority level, and any reflex testing instructions to prevent misunderstandings and avoid repeated specimen collection.

Clinical History

Relevant clinical details, medications, or exposure history that affect test interpretation or selection of additional assays by the laboratory.

Chain-of-Custody

Signatures or initials and timestamp fields for each transfer event to document custody, which is critical for forensic, legal, or regulatory review.

Configuring an Electronic Submission Workflow

Set these digital workflow options to mirror your paper process and reduce manual errors during eSubmission.

Field Configuration
Required Fields Mark patient ID, sample ID, and collection timestamp as mandatory.
Conditional Fields Show special handling fields when certain tests are selected.
Notifications Email alerts to lab intake on form submission and exceptions.
Integration Auto-pull patient demographics from EHR or LIS where available.

Technical Considerations for Digital Submission and eSigning

Choose a platform that supports secure upload, audit trail capture, and integration with lab information systems.

  • File Formats: PDF and DOCX are widely supported.
  • Integrations: Connectors for EHRs and cloud storage reduce rekeying.
  • Authentication: Email, SMS, or higher-assurance methods available.

Typical Timelines and Handling Deadlines

Different tests and sample types impose specific collection and transport time windows; plan based on assay stability and lab hours.

STAT or Urgent Tests:

Deliver within 2 hours; immediate intake required for time-critical results.

Routine Diagnostic Tests:

Deliver within 24–48 hours per lab guidance and sample stability.

Frozen or Cryogenic Samples:

Ship on dry ice with overnight service; document dry ice weight and hazard handling.

Ambient-Stable Samples:

Confirm acceptable ambient temperature window with receiving laboratory.

Rejected or Incomplete Forms:

Expect processing delays or rejection until missing data is provided.

Key Milestones from Collection to Result Reporting

Track these milestones to measure turnaround time, identify bottlenecks, and maintain responsibility at each stage.

01

Collection Completed

Specimen collected, labeled, and form initially completed.

02

Transport Dispatched

Courier pickup or internal transfer initiated; temperature controls verified.

03

Laboratory Receipt

Intake logs specimen, verifies identifiers, and assigns accession.

04

Testing and Reporting

Analytical work performed and results released to ordering clinician.

Common Preparation Errors to Avoid

  • Incomplete or missing patient identifiers cause specimen rejection and require recollection, delaying care.
  • Incorrect sample container or insufficient volume invalidates many assays and can force repeat collection.
  • Illegible handwriting on paper forms leads to transcription errors and misinterpretation of orders.
  • Failure to indicate special handling (temperature, light sensitivity) damages specimens during transport.

Essential Data Elements to Protect and Verify

Patient ID: Medical record or external identifier
Specimen ID: Unique barcode or accession code
Collection Time: Date and 24-hour time
Test Requested: Named assay or CPT code
Collector: Name and initials for custody
Handling Instructions: Temperature and biosafety notes

Operational and Compliance Risks from Errors

Sample Rejection: Delays in diagnosis or need for recollection
Misidentification: Wrong patient result risk and potential harm
Chain-of-Custody Gaps: Forensic or regulatory evidence may be invalidated
Billing Denials: Insurance claims may be rejected without correct documentation
Data Breach Exposure: Unauthorized PHI access risks HIPAA liability
Regulatory Sanctions: Licensing actions for repeated noncompliance
