SPORTS COMPETITION WINNER FORM & PRIZE AGREEMENT

RECITALS

WHEREAS, Organizer: (the "Organizer") conducted the competition described below; and

WHEREAS, Winner: (the "Winner") has been determined pursuant to the Organizer's official rules and is eligible to receive the prize set forth herein; and

WHEREAS, the parties desire to document the award of the prize, the Winner's obligations related to receiving the prize, and the terms governing payment, publicity, and release of claims.

Prize Description:

Late Payment: Organizer shall pay a late fee equal to of the overdue amount per month, computed daily, and the Organizer shall be responsible for reasonable collection costs.

SCOPE OF WINNER OBLIGATIONS

The Winner agrees to perform the obligations described below in connection with acceptance of the prize:

Such obligations may include attendance at award ceremonies, promotional appearances, photo and video releases, and submission of identification or tax documentation as required by Organizer for prize fulfillment.

TERM AND TERMINATION

This Agreement commences on Start Date: and continues until End Date: or until completion of all obligations, unless earlier terminated as provided herein.

Either party may terminate this Agreement for material breach by the other party if the breaching party fails to cure within days after written notice.

CONFIDENTIALITY

Each party shall hold in confidence nonpublic information disclosed in connection with this Agreement and shall not disclose such information to third parties except as required by law or with prior written consent. Confidential information does not include information that is publicly available through no fault of the receiving party or independently developed by the receiving party.

RELEASE AND INDEMNITY

By accepting the prize, Winner releases and forever discharges Organizer, its affiliates, agents, employees, and sponsors from any and all claims, liabilities, actions, or causes of action arising out of or relating to the competition, the awarding, acceptance, possession, use, or misuse of any prize, including but not limited to personal injury, property damage, or publicity rights, except where such claims arise from Organizer's gross negligence or willful misconduct.

Winner agrees to indemnify and hold harmless Organizer from any third-party claims arising from Winner's breach of this Agreement or from Winner's negligent acts in connection with prize activities.

Acknowledgement of Release: I acknowledge and accept the release and indemnity provisions above.

PUBLICITY

Winner grants Organizer a perpetual, royalty-free license to use Winner's name, likeness, voice, and biographical information in connection with promotional materials, announcements, and publicity relating to the competition, in all media now known or hereafter developed, unless Winner specifically opts out in writing prior to execution of this Agreement.

Opt-out: I elect to opt out of publicity uses (note: opting out may affect prize eligibility or delivery).

GOVERNING LAW

This Agreement is governed by the laws of the State of without regard to its conflicts of law principles. Venue for any dispute arising under this Agreement shall be the state or federal courts located in the specified state.

ENTIRE AGREEMENT

This Agreement, together with any attachments and the Organizer's official competition rules incorporated by reference, constitutes the entire agreement between the parties concerning the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings. Any amendment must be in writing signed by both parties.

CERTIFICATIONS

By signing below, Winner certifies that all information provided is true and complete, that the Winner is eligible under the competition rules, and that the Winner will provide any documentation reasonably requested by Organizer to verify identity and eligibility. Winner acknowledges potential tax reporting obligations and accepts responsibility for any tax liabilities arising from receipt of the prize.
