WHEREAS, Organization operates sports programs, training sessions, practices, camps, clinics, and competitive events designed to develop athletic skills and provide participant recreation and competition; and

WHEREAS, Participant desires to participate in Organization's sports activities on the terms set forth in this Agreement and Organization agrees to provide such activities subject to Participant's compliance with rules, safety protocols, and payment obligations; and

WHEREAS, if Participant is a minor, the undersigned Guardian consents to participation and agrees to the terms, releases, and indemnities contained herein.

Risk Acknowledgment, Release and Indemnity

Participant acknowledges that participation in athletic activity involves inherent risks including bodily injury, aggravation of pre-existing conditions, and property damage. Participant knowingly and voluntarily assumes all such risks. Participant releases Organization, its officers, coaches, employees and agents from any and all liability for injury, illness, death, or property loss arising from participation except where caused by Organization's gross negligence or willful misconduct.

Participant agrees to indemnify and hold harmless Organization from and against any claims, demands, damages, losses, costs and liabilities (including reasonable attorneys' fees) arising out of Participant's negligent or intentional acts or omissions in connection with participation.

Participant will not disclose, reproduce, or use such confidential materials for any purpose other than participation under this Agreement without Organization's prior written consent. This obligation survives termination of the Agreement for a period of two years.
